  4. Delivering Proposals • If delivering proposals in person, please consider traffic, parking and security check in. • Proposals must be received in the Office of Procurement Services by 11:00 a.m. ET on June 4, 2014. Proposals received after this will not be considered. • If using a delivery service (FedEx, UPS, etc.) be sure to check delivery times and schedule.

  5. Addendums • If any changes are made to the RFQ, an addendum will be posted on the APS website. • Please, check the website daily for any addenda. • Vendor questions and answers will be posted as an addendum.

  6. ProcurementProcess • 4.1Communication • All communications regarding this solicitation must be with the assigned Procurement Officer, David Odom. • 5.0 Background Check • APS policy requires that all contractors, consultants, or vendors providing services on APS premises be fingerprinted and submit to a criminal record check initiated by APS prior to providing services to APS. There is a fee of $ 44.25 per individual payable by money order associated with the background check.

  7. Required Documents Non – Submittal Response Forms • Complete and submit the following forms with your proposal if applicable: • Offeror affirmation form • Offeror information form • Joint venture affidavit (if this will not be a joint venture, check the appropriate box on the form and return with your response) • Primary vendor / subcontractor utilization (if subcontractors will not be used, check the appropriate box on the form and return with your response) • Promise of non-discrimination • Contractor Affidavit of Compliance under O.C.G.A 13-10-91(b) (1), if applicable • Subcontractor Affidavit of Compliance under O.C.G.A 13-10-91(b) (3), if applicable. • Sub-subcontractor Affidavit of Compliance under O.C.G.A 13-10-91(b) (4), if applicable • Affidavit of Exception, if applicable. • Copy of local or state business license or permit • Signature page to APS contract

  8. Submission Requirements 6.0 SUBMISSION REQUIREMENTS6.1 Proposals shall be submitted as follows:Section 6.12, Section 1, Company Qualifications and Experience and Section 6.12, Section 2, Methodology / Operations.1. The original (marked as such) documents and five (5) hard copies of all documents in a separate sealed envelope labeled Section 1 and Section 2.2. All hard copies should be submitted on loose leaf paper, front only.3. All documents saved on three (3) CD-ROMs or Flash Drives. All CD-ROMs/Flash Drives should be labeled with the company name, bid name and bid number. All information on CD-ROMs/Flash Drives should be provided in a single PDF file format.

  9. Contract Administration 8.0 AWARD 8.2 This contract award is for one (1) base term (of one year or less) with four (4) one-year options to renew at the sole discretion of the Superintendent of APS. • 9.0 CONTRACT • 9.1 By submitting a response to this solicitation, the offeror affirms that it has read and will accept all provisions of the contract as written and attached to this solicitation. Offeror also understands that these provisions are non-negotiable and that the contract is not legally binding upon the parties until executed by APS and offeror. • 9.2 The offeror must sign and submit the contract signature page with the response to the solicitation. By doing so, the offeror acknowledges that it accepts the provisions of the APS contract as written. • 9.3 Once a vendor has submitted a quote and been selected to provide a professional learning session, a detailed statement of work with deliverables and pricing for that session will be executed by APS and the vendor.

  10. Scope of Work • 3.2 APS will establish a pool of vendors to provide professional learning services on an as needed basis in one or more of the following categories: • Assessment Strategies, Practices and Uses • Content Knowledge of Instructional Staff • Establishing and Supporting Communities of Practice and Professional Learning Communities • Establishing and Sustaining a Culture of Achievement and High Expectations for all • Leadership Practices that increase Student Achievement • Meeting the Needs of Diverse Learners • Researched, Instructional Best Practices that Support Student Achievement • Standards-based Grading Practices • Standards-based Instructional Practices and Strategies • Understanding, Teaching and Assessing the Common Core Standards • Use of Data • Creating Engaging and Rigorous Instruction in High-Poverty Schools • Classroom Management Strategies
