  2. Introduzione: Recentemente è stato descritto un distinto gruppo di tumori renali che insorgono in reni con malattia renale terminale. Cellule con citoplasma chiaro Struttura papillare CK7 CK7+ AMACR - AMACR Tickoo et al. Am J Surg Pathol. 2006

Introduzione: La corretta classificazione delle neoplasie a cellule renali oggi può contare su specifiche indagini immunoistochimiche e citogenetiche in grado di permettere la distinzione di entità neoplastiche con caratteri morfologici talora sovrapponibili. Materiali e Metodi: Abbiamo individuato e studiato 7 tumori renali con le stesse caratteristiche morfologiche di quelli sopradescritti ma che insorgevano anche in reni senza malattia renale terminale • Immunoistochimica: • CK7 • Anidrasi Carbonica IX • AMACR • CD10 • TFE3 • FISH: • CEP 7 • CEP 17 • CEP Y • CEP 3 • 3p25

Conclusioni: • Il CARCINOMA RENALE A CELLULE CHIARE PAPILLARE: • può insorgere anche in reni senza malattia renale terminale. • ha un profilo immunoistochimico e un assetto citogenetico distinti da quelli visti tipicamente nel carcinoma renale papillare e nel carcinoma renale a cellule chiare • dovrebbe essere considerato un’entità distinta nello spettro delle neoplasie renali.

CLEAR-CELL PAPILLARY RCC E’ UNA NUOVA ENTITA’ NEOPLASTICA • Architettura prevalentemente papillare • Cellule chiare • No necrosi-No macrofagi-No Psammoma bodies • Caratteristica disposizione dei nuclei a basso grado MORFOLOGIA • CD10 negativo • AMACR negativa • CK7 negativa • TFE3 negativo IMMUNOISTOCHIMICA • Cromosoma 7-17-Y numericamente normali • Cromosoma 3p non deleto FISH
