  3. Recommended Exams • Medical, Dental and Eye exams are encouraged (routine check-ups) • Forms included in packets

  4. Health Alerts • Health history (obtained from parent/physician input) updated yearly and “as needed” or requested • Shared with school staff on a “need to know” basis • Determine if it is a Health alert or IHP (individualized health plan) • IHP developed between school nurse and parent • Indicates health condition/illness • Specifies school implications and or emergency measures needed • Identifies parent contact information (important to keep current)

  5. Medications • School Policy • Medications must be brought to school in original labeled container with student name • Medication authorizations • Prescription medication require parent and provider signature • Over the counter medications require parent signature • Forms available in school office and district website

  6. Immunizations • Mandatory state requirement • Specific guidelines established each year • Resources: • Wisconsin immunization Registry (WIR) • Communicable Disease Center (CDC) • Brown County Health Dept. (BCHD) Immunization clinics • Waiver • Notification of immunizations to Pupil services secretary 662-7881

  7. Staff • District nurses available throughout district and by office, cell phone contact. • Secretaries and special education aides trained in; • American Heart CPR, AED, and First Aid • Medication administration • Blood borne pathogens precautions
