CIDP診断 IVIg 単純血漿交換療法 副腎皮質ステロイド療法 禁忌疾患あるいは病態 チェック 単純血漿交換療法 40 ml/kg 血漿処理 最初3週間 週2回 その後3週間 週1回 IVIｇ禁忌　あり 単純血漿交換療法禁忌　なし IVIg禁忌　なし IVIg 0.4 g/kg/日 5日間 治療開始後2週間以内 効果判定 IVIｇ禁忌　あり 単純血漿交換療法禁忌　あり 副腎皮質ステロイド療法禁忌　なし 無効 有効 無効 治療開始後2週間以内 効果判定 診断を再考 有効 副腎皮質ステロイド療法 禁忌病態 なし Prednisolone 1 mg/kg 体重/日 連日4週 あり 初回IVIg開始日から 4週後 2回目IVIg 0.4 g/kg/日，5日間 8週後 3回目IVIg 1 g/kg/日，1日間 無効 5 mg/週ずつ漸減 15 mg連日の翌週から20 mg 隔日 1年経過した時点で5 mg/4週ずつ漸減 中止 Cyclosporin禁忌病態 なし 無効 Cyclosporin 5 mg/kg 体重/日 連日 Cyclophosphamideパルス 1 g/m2 体表/日 1日間 有効 有効 少なくとも1年間継続 最長6ヶ月間継続 図 1.
