  2. RADIATION SAFETY RECORDS Records in support of the requirements described in the 2003 RSM must be maintained in the laboratory. These records must be available for review by the UGA/RSO or regulatory agencies. It is recommended that records be organized in labeled binders for easy access and to facilitate the review process. Examples of these records include: Radioactive materials permit application, approval and amendments Training Records Monthly Radiological Survey Records Sewer Disposal and Waste Records RAM inventory summary and radioisotope forms

RECORD RETENTION RETAIN FOR DURATION OF PERMIT • Original application, approval and amendments. • Laboratory Radiation Safety Program • Security and Safety Tests • Training records • RETAIN FOR 3 YEARS • Contamination Surveys of each Lab and Source • Radioactive Materials Inventory • Waste Disposals

  5. Radioactive Materials Permit(RMP) Application

  6. Completed Radioactive Materials Permit Classifies you as anAuthorized User under UGAsRML

  7. Chronological Approved Amendments • Possession limits • Change of Location • Inactive or termination status • ARW changes • Sewer Discharge permit

  8. RWC ! • Keep a copy for your • Records ! • Send us a copy for our • rad worker file NOTE:This form must accompany the Dosimetry Request Form when applying for a dosimeter badge

  9. Records must contain an on-line certificate for every radworker for each year working under a RAM permit

  10. WHITE COPY IS YOUR RECORD YELLOW COPY IS SENT TO THE RSO FOR REMOVAL OF YOUR USED OR DECAYED ISOTOPE IF YELLOW COPY IS LOST, A PHOTOCOPY WILL SUFFICE

  11. ISOTOPE TRACKING SUMMARY FORM MUST BE KEPT UP TO DATE STATE INSPECTORS WILL LOOK AT THIS FORM

  15. EACH LIQUID WASTE CONTAINER MUST HAVE THIS FORM EACH FORM SHALL BE INITIATED WHEN THE FIRST WASTE IS PLACED IN THE CONTAINER ANALYZED WHEN FULL OR WHEN YOU WANT TO DISPOSE FORM MUST BE COMPLETED WHEN WASTE IS RETRIEVED MAKE COPY FOR YOUR RECORDS

  16. EACH DAW CONTAINER MUST HAVE THIS FORM EACH FORM SHALL BE INITIATED WHEN THE FIRST WASTE IS PLACED IN THE CONTAINER COMPLETE WHEN FULL OR WHEN YOU WANT TO DISPOSE FORM MUST BE COMPLETED WHEN WASTE IS RETRIEVED MAKE COPY FOR YOUR RECORDS

Know the difference between RADIATION and CONTAMINATION Practice and use ALARA fundamentals of TIME, DISTANCE and SHIELDING There is a risk in everything we do, working with radiation only increases your risk of health effects slightly. Remember you are only receiving chronic doses if any. Use your GM ! Every permit carries the responsibilities ofperforming, documenting and reporting radiological surveys, waste disposal, inventory, training and records management.
