  2. Section 1 – Lab Safety Plan and Annual Training: • Provides link to Lab Safety Plan templates • Outlines that annual safety training is required • Provides link for self-reporting of training

  3. Section 2 – Anesthetic Gases • Little change from the past • Now has a blank for delivery and scavenging methods that must be filled in by researcher • Option checklist has been removed

  4. Section 3 - Chemicals • Major changes • Now asks if fume hood available • Lists some examples of chemicals and pharmaceuticals that must be listed • Has a link to DEHS website with full text info on high hazard chemicals • Directly asks for SOPs for each agent

  5. Section 4 - Radiation • Only change is addition of asking for safety procedures for animal care staff

  6. Section 5 – Human blood, etc. • Little change • Asks if human tissue, cells, blood, etc. being used in protocol • Still has note that Universal precautions must be followed when handling human cells/tissues/blood

  7. Next is a note stating that any affirmative answer to sections 6 through 8 requires an application to and approval from the IBC

  8. Section 6 – Infectious agents • Little change to the table • Below table is a blank to enter the IBC code and project title

  9. Section 7 – Biologically-derived toxins • Set up similarly to infectious agents. • Similar table • Same blank below for IBC code and project title

  10. Section 8 – Recombinant DNA • Now only asks if used • Then asks for IBC Code # and Project title
