  3. Dental Plan Highlights • Employers with 2+ employees • Groups with 10 or more enrolled can select from triple dental option • Groups over 100 employees may offer dual PPO Networks, Dentemax, Maverest and PPO USA/ Connection • Annual Maximums - $750, $1000, $1500 or $2000

VISION PROGRAMS Routine eye exams provide early detection of disease such as Diabetes, Hypertension and Glaucoma Early detection means reduced medical costs and less time off from work PROVIDER NETWORK SERVICES • Extensive national provider networks including independent practitioners and retail chains such as Wal-Mart, Sam’s Club, Costco, JCPenney, Sears, Pearle Vision, Target Optical, Eyemasters, Ossip Optometry and more • Freedom of choice - Members may select any eye doctor or receive discounts from our extensive national network of participating providers • Use benefits online to order contact lenses

Vision Plan Highlights • Employers with 2+ employees • Exam copayment • Exams once every 12 or 24 months • Frames once every 12 or 24 months • Lenses or Contact Lenses once every 12 or 24 months • National PPO Network – Private Practitioners and Retail Chains, (including Wal-Mart, Sam’s, JC Penney, Costco, Sears, Pearle Vision and Target, but not limited to)
