2. Cherry Point Refinery Fire • On 02/18/2012, fire broke out in the crude processing unit; cause of the fire remains under investigation • Ecology requested assistance of EPA to conduct ambient air monitoring

3. Cascade Petroleum TankTruck Rollover • On 03/12/12, a tanker truck with trailer overturned on US Highway 58 releasing about 4,200 gallons of diesel and gasoline into a tributary of the Middle Fork of the Willamette River • Incident affected soil, surface water, and groundwater • Responsible party performed cleanup action with federal and state oversight

4. Repsol Q2 Pad – Gas and Mud Release • On 02/15/12, an exploratory well being drilled on Alaska's North Slope experienced a blowout when drillers encountered a gas pocket approximately 2500 feet below the surface • Approximate 42,000 gallons drilling mud and an undetermined volume natural gas was expelled from the well onto the gravel pad and nearby tundra • The well was abandoned on 03/15/12

5. Stubblefield Salvage and Recycling • Former metals salvage facility • 10/09 EPA initiated Phase I of a multi-phase removal action • 21 55-gallon drums containing hazardous substances and/or oil, 40 yd3 of PCB contaminated debris, 75 yd3 of lead contaminated soil, and 150 yd3 of asbestos contaminated materials were removed • 04/12 EPA returned to complete Phase I - 61 55-gallon drums containing hazardous substances and/or oil were removed

6. 11th Street Chemical Fire • NW Chemical Recycling repackaged bulk chemicals for purchase on eBay, including explosives, flammable liquids, oxidizers, radioactive materials, and reactive metals - On 03/28/12, a fire and explosion erupted due to tenant repackaging sodium chlorite from a bulk container to a smaller container • EPA conducted a removal action between 03/29 /2012 and 04/18/2012 • 7,000 to 10,000 laboratory grade chemicals in more than 1600 various sized containers were overpacked • Approximately 320 cubic yards of burnt building debris and containers with chemical residue

7. Jimmy Creek Ranch • The Jimmy Creek Ranch treated fence posts with pentachlorophenol (PCP).  - Oil-stained soil and residual wood-treating product burn pits • At the request of ODEQ, EPA conducted a multi-phase removal action • 11/2011. Oversight of RP excavation of contaminated soil and sampling activities • 03/2012. Oversight of additional RP excavation of contaminated soil and sampling activities

8. Bend Chemical Storage Locker • Numerous totes and pails of assorted lab chemicals and waste were found in a storage locker and industrial unit at a storage facility • EPA conducted a removal action between 03/12 – 15/2012 • Approximately 400 different laboratory grade chemicals in different sized containers were overpacked

9. McCuthchen Containers • Abandoned chemicals left on the street corner of a private business near Seattle, Washington • Requested by the State and City Utility to assess and dispose of the unknown substances • Chemicals were assessed and, in cooperation with the County, were taken to the local household hazardous waste collection area for disposal
