  1. Evaluation of Key Information Summary (KIS) in Forth Valley David Arundel Programme Manager

Background • Over 2000 patients currently on ACP across 3 care groups, care homes, palliative care, LTC • For ACP’s to be effective • Ability to transfer information to other care providers is essential element in making this happen • FV had a bid for eHealth funding to support long term conditions programmes approved and became one of 4 eHealth demonstrator sites evaluating the KIS (2 EMIS, 2 Vision)

  3. Objectives and Outcomes • To evaluate the Key Information Summary (KIS) across a sample of EMIS practices (n=11) and to test the ability to easily share information on the GP system and ensure that it is viewed and understood by users in A&E (EDIS), OOH (ADASTRA), SAS, NHS24. • Outcome – An report that that will demonstrate; • Views of EMIS practices on the usability of KIS • Whether KIS can be viewed on other users systems • Feedback from recipients on the quality and usefulness of the information • Impact of the information on patient outcomes.

  4. Deliverables • KIS electronic template made available from EMIS developers • Training for participating practice staff • Evaluation methodology developed and agreed • Final project evaluation report with key findings and lessons learned is submitted to National Programme Board • Recommendations along with other Boards piloting KIS to inform final KIS template • FV participation in information and launch events for local and national roll-out.

  5. Key Timelines • Meeting with GP practices – 1st Dec • KIS user interface available – Jan 2012 • KIS training in participating practices – Feb 2012 • Adastra implementation – ?expected Feb / Mar 2012 • KIS ‘live testing’ and evaluation – March / April 2012 • National rollout starting May !!

Progress to date • Local Project Board established • PID document developed • Practice recruitment completed • FV staff involvement in Webex demonstration of screenshots and SEF testing • Project monitoring thro KIS Project Board • Highlight reports to CHP eHealth group • Practice funding released December 11 & March 12 • Raising awareness of KIS pilot

Practice requirements • Training on KIS for participating practices • Transfer existing patients’ ACP onto KIS / create KIS for new patient (minimum n=5) • Obtain patient consent to • Participate in pilot • Share KIS with others • Provide patient with copy of patient leaflet • Provide patients with a print of their KIS

  10. Practice requirements • As a minimum both a GP and a nurse are involved in completing the KIS • Practice Team to participate in evaluation and contribute to evaluation report • Practice Team to attend meetings (pre and post) evaluation

Evaluation • Supported by QI • Developing evaluation methodology • Informing patient of pilot (amend leaflet for pilot) • Information viewed by HCP as part of pilot • Drafted evaluation questionnaire (usability, accessibility and usefulness) • Maintaining issues log of glitches • Seeking Caldecott permission for process
