Service Activities THUMBS UP THUMBS DOWN Activities for school sports and clubs, with the exception of service clubs Religious activities involving education, evangelism, or worship Activities for organizations that are not identified as non-profit, unless the activity is education related Services that directly benefit a member of your family Court ordered service to the community • Activities that serve an educational institution but are not tied to a grade or other compensation • Activities for non-profit organizations • Activities that serve an individual in need who cannot otherwise provide for himself

Date; Month and Day Service Organization; Official Name Job Responsibilities; List Supervisor’s Title and Signature Number of Hours • Student Name; First and Last • School; Fairfield Middle School • Teacher Name; Homeroom Teacher • Grade
