  4. Medical Evaluation • Prior to being issued a respirator • Conducted by a professional medical doctor • Examination is kept confidentially • Duration and frequency of respirator use • Any limitations on respirator use • Medical evaluation kept on file

  5. Fit Testing • Must pass a fit test prior to wearing respirator • Qualitative fit test for half face respirator • Quantitative fit test for full face respirator • Issuance form must be filled out • Positive and Negative fit-check

  6. Maintenance and Care • Visual inspect respirator prior to use • Cleaned after each use • Stored in a clean and sanitary location • Repairs are made according to manufacture
