  1. ISCAN Pathway Child & Family – Concerns about child with additional needs/disabilities Referral sent using ISCAN form to Caerphilly Children’s Centre Administration & referral logged and actioned at Caerphilly Children’s Centre – Acknowledgement sent within 2 working days Referral meets criteria – Complex additional needs Within 2 working days of referral being received Information & Advice Officer makes contact with Family or referrer where appropriate to check consent and answer any questions about the ISCAN Within 7 working days ISCAN Multi-Agency Referral meeting takes place to discuss case(s) – where relevant a Lead Professional is allocated, if criteria is not met, signposting to other services may take place Within 15 working days of allocationLead Professional arranges & completes initial information gathering assessment & arranges a date for the TAC meeting (using the TAC Request form Caerphilly Children’s Centre admin are notified of the date of the TAC meeting Lead Professional takes or sends completed assessment to the ISCAN Multi Agency Panel for outcome and actions to be discussed. Assessment is signed off by Multi Agency Panel TAC meeting is convened by the Lead Professional within 30 working days from the date of allocation of the Lead Professional If a different Lead Professional is required, this will be referred to the Multi Agency Referral meeting to allocate TAC Meeting produces the Family Service Plan/Action Pan – Regular TAC meetings take place (between 3 – 6 months depending on needs) Lead Professional takes or sends completed Family Service Plan/Action Plan to ISCAN Multi-Agency Panel to be discussed and signed off by Multi-Agency Panel Multi Agency Referral Meeting – review, monitoring and evaluation of cases Project Board Strategic/Auditing Function ***Exit Criteria - When needs have been met, or family no longer requires co-ordination of services the chid & family will be discharged from the service*** ****Discharged ISCAN family/Professional can request ISCAN TAC review – if they there is a concerns or can self-refer back to ISCAN**** *****Discharged cases to be discussed at Multi—Agency Panel for discharge agreement*****
