Using the Outcome Tool • Explain to the group the reasons we need to do this (briefly) • Give a quick explanation of each score • Give a quick explanation of each area and the type of things they need to be thinking of with each question. • They complete the form. • Follow up after looking through the forms under the additional comments section. www.depauluk.org

Completing the goal finder/support plan • Each section is linked to an area of the star. • Each section asks questions that are linked to the project we are delivering • At the end of each section ask the young person where they would place themselves on the outcome tool • Repeat on a regular basis www.depauluk.org
