Voluntary Short-Term Disability Income Insurance (Day 1 - 90) • Paid for by the employee • Benefit: Up to 66 2/3% of gross monthly wage (paid weekly) • Elimination Period: Accident: 14 calendar days Sickness: 14 calendar days • Maximum Benefit: $4,000 monthly ($923.00 weekly) • Maximum Benefit Period: Up to 12 weeks • Sick time can be used in addition to STD payments

Employer Paid Mid-Term Disability Income Insurance (Day 91 - 180) • Paid for by Maricopa County Community College District • Benefit: 66 2/3% of gross monthly wage (paid weekly) • Elimination Period: Accident: 90 calendar days Sickness: 90 calendar days • Maximum Benefit: $4,000 monthly ($923.00 weekly) • All leave accruals must be exhausted before Mid-Term Disability pays • Maximum Benefit Period: Up to 12 weeks (after 1st 90 days)

Open Enrollment Rules • New Hires are eligible to elect up to $1,450 monthly benefit with no Underwriting (Statement of Health), if salary allows and if election is made within the first 31 days of becoming eligible. • Late entrants (Employees who decline coverage when first eligible) will be subject to Statement of Health for all coverage amounts. • For those currently covered under the STD policy, during open enrollment periods a one-step increment increase is allowed per fiscal year, if salary allows an increase, without a Statement of Health.
