California Community Choices Grant California Health and Human Services (CHHS) requested proposals for their California Community Choices Grant project.  In 2008, CHHS awarded the California Community Choices Grant to CalOptima, in partnership with the County of Orange Office on Aging, and the Dayle McIntosh Center. The purpose of the grant is to develop a long-term care infrastructure to increase access to home and community-based services and to help divert persons with disabilities and older adults from unnecessary institutionalization.

  5. ADRC in Orange County • CHHS awarded CalOptima a 3-year grant (5/1/08-4/30/11) in the amount of $337,519 to develop the Aging and Disability Resource Connection (ADRC) in Orange County. Formed a Partnership Between: • CalOptima • Office on Aging • Dayle McIntosh Center

  7. Target Population • The ADRC program will target: • Seniors age 60 and over • Persons with disabilities of all ages • Caregivers • Health and long term care professionals • Others who provide services to seniors and persons with disabilities

ADRC Program Goals • Develop a “one-stop” system of relevant, accessible, and up-to-date information and provide counseling on long-term support services and eligibility determination for community programs/services • Promote collaboration: linkages between and among consumers, caregivers, and health and social service providers • Increased awareness of long-term support services to empower consumers to make more informed decisions about their care

Stakeholder Input • CalOptima will involve the entire community in the development of the ADRC program by: • Convening an Advisory Committee, next meeting 4/9/09 • Convening a series of community forums and demonstrations to provide outreach and education to the target population. • Disseminating information on the ADRC program through print media, newsletters, press releases, etc. • Future input: Surveying the target population to assess overall satisfaction with the ADRC program.
