  6. How does it work?

  7. Get a form from the D9500 website or from your secretary

  8. What do you do? • Complete the form – ensure you give all the requested info • Decide on how you want to pay – cheque or credit card • Decide how much you want to contribute – minimum $100 • Give form to club Centurion Officer or post to address on form to Team Centurion

  9. What happens then? • Donations are recorded on the Team Centurion database • The donations and forms are then sent to Parramatta once a month – copy kept at Team Centurion • Parramatta sends a tax deductible receipt to the donor via the club on a monthly basis • Team Centurion issues a certificate to donors via the club • Team Centurion also issues a Centurion Club lapel badge to first time donors via the club

  10. Certificates/lapel badges are posted to your secretary
