Processing of a Class 3 (including Class 1and 2 activities) risk assessment Risk assessment prepared1 Risk assessment submitted to Site BSO Subject to comments, Class 1 elements of work may commence Local GMSC must convene to review assessment Assessment with GMSC comments to Chair of University Committee on Biological Hazards Formal note sent to PI with feedback from GMSC Risk assessment signed by PI2 , HoS/HoD and Chair of CoBH Risk assessment submitted with notification to HSE Original signed copy of assessment to be retained by PI, with copy sent to BSO Subject to comments from HSE, Class 2 elements of work may commence UBSA receives acknowledgement from HSE Request for additional information from HSE – USBA prepares in consultation with PI (and GMSC if appropriate) Subject to comments from HSE, Class 3 elements of work may commence Consent received from HSE Risk assessment amended if required 1Assessment can be prepared by researchers involved in the project but must be under the supervision of the PI 2Only permanent members of staff can sign assessments
