  3. Statewide Equipment Program (SWEP) • Continence Aids Payment Scheme (CAPS) • Department of Veterans’ Affairs (DVA) • Disability Continence Support Service Funding schemes

  4. Some continence products may be available on an individual basis as part of a care package as negotiated with case managers through programs such as: • Linkages • Disability Services Other Options to Explore

  5. State funded scheme – http://swep.bhs.org.au • Community based • Excludes funded residential care facilities • Washable and reusable products • Requires a continence assessment by a registered prescriber State Wide Equipment Program SWEP

  6. Federally funded scheme http://www.bladderbowel.gov.au/caps/ • Managed by Medicare • Paid directly into bank account • Application form – health professionals • Certain eligibility criteria • +/- Pension concession card • Clients 5 years and above • Any products Continence Aids Payment Scheme CAPS
