  1. Health Information Exchange (HIE) What the clinician will see when they click Community View in PowerChart

  2. How will clinicians access the HIE information? • At University of Missouri HealthCare the clinician will open the patient’s chart and click on the “Community View” Menu selection in PowerChart.

  3. Patient Data in This Presentation • Information displayed in the following screen shots is not associated with a real patient. • Information is just a mock up for training purposes.

Disclaimer on the Page • This record is an aggregate summary of medical information from multiple participating healthcare providers. • It is intended to support optimal patient care. • It is NOT intended to replace the patient’s medical record. • It may not include all historical information on the patient.

  6. Navigation Menu The Navigation Panel: * shows all the sections of the record * allows you to navigate on the screen

  7. Tools Menu The Tools Menu allows you to * search the screen * increase or decrease the font size

  8. Demographic Bar The Demographic Bar shows you: * Patient Name * Sex * Age * Date of Birth (DOB) * HIE ID Unique number HIE assigns to patient * Primary Care Physician (PCP) Comes from latest ADT not necessarily UMHC ADT * Home phone Comes from latest ADT not necessarily UMHC

  9. Page Search Type in a key word and click the Search button to search for information contained on the page. If information is found it will display in yellow.

  10. Conditions/Problems The Conditions/Problems section shows you: * Current problems pulled from the Problem List * Displays the source of the data (e.g. CRMC – Capital Region Medical Center) * Hover over the source name to see the name of the facility

  11. Medications The Medication section shows you: * Current medications (both prescriptions and historical/home meds) * The Search window in this section only searches the medication portion of the window and not the whole page. * All searches work the same except for the Page Search which searches the whole page.

  12. Allergies The Allergies section shows you: * Current allergies

  13. Lab Orders The Lab Orders section shows you: * Labs for the last 6 months (default) To see more data, click on the links – for example Past Year

  14. Immunization/Vaccine History The Immunization / Vaccine History section shows you: * Immunizations and date given

  15. Microbiology / Anatomic Pathology If there are No Results the section will display in blue type.

  16. Vital Signs If results are outside of the reference range they will display in red. Click the link to see Last 6 Results.

  17. Vital Signs Graphing Click the link under Weight to graph the results. Any item that has a link can be graphed.

  18. Radiology Report Images are not viewable through the HIE only reports.

  19. History & Physical Results History & Physical Results will display as reports. Images are not viewable through the HIE.

  20. Radiology and H&P Results Radiology and History and Physical Results will display as reports. Images are not viewable through the HIE.

  21. Lab Results Click on the links to graph lab results. Low and High values will display in red.

  22. Lab Results – Changing the Display Click on the Switch link to change the display to see the Order Name, Reference Range and to view more links to past data (e.g. Past Year instead of the 6 month default display)

  23. Discharge Summary & Provider Reports Provider reports may have different nomenclature – for example a Cath Note may be called an Op Note by some organizations or a Diagnostic Report by another organization. EKG’s tracings won’t be in the system but interpretations will be in the system.

  24. Visits Displays location of the visit (e.g. Private Clinic) and shows if the visit was an inpatient or outpatient visit, the reason for visit and the attending provider.

Benefits of the HIE • Gives providers immediate access to help treat the patient. • Reduces the need for duplication of tests and treatments because the provider can see the patient’s results. • Provides information to help reduce the chance for medication and allergy interaction errors • Maintains the privacy of the patient’s health information while making it available when and where it is needed as long as the provider is in the exchange and the patient has given written consent.
