  2. Utilizing ThisTool This is a self guided training tool provided to any surveillance site to familiarize individuals with reporting requirements established in Ohio Administrative Code 3703-3-01 through3701-3-31

  3. WHOis considered a surveillancesite? • Ohio reportable disease reporting requirementsoutline • WHOmust reportincluding: • Healthcare providers(physicians, hospitals, infection preventionists) with knowledge of a case or suspected case of a disease which must bereported • Laboratoriansthat examine specimens of humanorigin • with evidence of diseases which are required to bereported • Any individualhaving knowledge of a person suffering from a disease suspected of beingcommunicable

  4. WHAT must bereported? • Diseases on Ohio’s ABC list (see nextslides)

  6. WHENmust a report bemade? • Class A disease –immediatelybyphone • Class B disease – by the end of thenext businessday • Class C disease – by the end of thenext businessday

  7. WHERE must the report bemade? • Healthcare providers and laboratorians should report to the local health jurisdiction in which the case or suspected caseresides • If the residence is unknown, report to the local health jurisdiction in which the healthcare provider or laboratory islocated • If the residence is outside of Ohio, report to the state of patientresidence

WHATinformation is needed fora report? • Healthcareproviders: • Name of case or suspectedcase • Diagnosis or suspecteddiagnosis • Date of birth of case of suspectedcase • Sex of case or suspectedcase • Telephone number of case or suspectedcase • Street address including city, state and zip code of caseor • suspectedcase • Supplementary surveillanceinformation • Healthcare provider name, telephone number and street address

Laboratorians: • Name of case or suspectedcase • Date of birth of case or suspectedcase • Sex of case or suspectedcase • Street address including city, state and zip code of case or suspectedcase • Healthcare provider name, telephone number andstreet • address • Laboratory testinginformation • Specimen identificationnumber • Specimen collectiondate • Specimentype • Test name • Test result • Organism and serotype, ifpossible

HOWmust a report bemade? • Class A – immediately by phone, follow-up with HEA 3333 as instructed by health jurisdiction; follow up can also be done through direct entry into Ohio Disease Reporting System(ODRS) • Class B and C – HEA 3333; Class B and C can alsobe • done through direct entry intoODRS Labs that report 200 or more Class A,B or C diseases each year should be working towards Electronic Laboratory Reporting (ELR)

  12. Class A After-HoursReporting: • TCHD has 24/7 capability for class A reporting and public healthemergencies • After-hours, weekend and holidays call (330) 343-5555 and listen totheprompts. You will be given another number to contact and be asked to leave a name and call backnumber • A qualified staff member from TCHD will return your callpromptly
