  1. Gift Card Fundraiser Purchase your Holiday gift cards now – and help the 5th grade at the same time! There is a gift card for EVERYONE! Relatives, teachers, co-workers, friends! WHAT IS IT? 1. Purchase a gift card at face value 2. A % of that sale goes to the 5th grade committee 3. % amounts are listed on the order form TOP OFF the Holidays! Order by Nov. 26th Pick up Dec 14th TO DO: 1. Take orders and collect $ 2. Complete the order summary form 3. Return summary form on 11/26/11 4. Include a check to the QH PTA for the FULL amount of your order FINAL STEPS: 1. Remember to submit your order on time! NOVEMBER 26th! 2. Pick up your gift cards December 14th. 8:30-9:30 am or 3-4pm in the school lobby. If you need to pick up at a different time, contact, Lisa Cote, klinkct@snet.net or 860-444-6955 3. Deliver your gift cards to those that ordered them. 4. Enjoy the Holidays without any shopping craziness!
