Priority groups More than 4 medicines daily (polypharmacy) Older People Care home residents Mental health issues People with learning disabilities Minority ethnic groups with English as an additional language Medicines that require additional or special monitoring People whose care is transferred from one setting to another or who have multiple carers or prescribers Long-Term Conditions
