One died on scene and the other diver was pronounced dead at the hospital. • 19 Feb 2013: (Oahu, HI) E-6 died during a planned open ocean swim evolution. • 14 Feb 2013: (near Catalina Island, CA) During submarine rescue system training, the Pressurized Rescue Module (PRM) fell damaging the PRM, transfer skirt, deck cradle, and umbilical. • 17 Jan 2013: (Philippines) MCM ran aground operating in the Sulu Sea near Tubbataha Reef.
