  4. Certificated Site Summary Overall Rollup* *Effective date: 1-Oct-2013. Includes the latest certificates of all suppliers that have a status of Certified or Suspended, or Expired with an expiration date less than 13 weeks prior. ( ) = 1-Apr-2013.
