  11. 14.10 : Initial Family Evaluation Admin Reqs • Initiate FE after receipt of the Caregiver Application • 8 Weeks To Complete • After all components are done • 3-In-Home Consultations • Assessors Requirements • IMPACT Trained • Case Manager Level or Approved Contractor • Director / Designee Approval • IMPACT Overview • Assessments terminated if caregivers does not submit all documentation within 60 days of initiation of FE.

  12. 14.11 Initial FE Components • General Requirements—the same ( medicals, drug screens, safety checks, written assessment and recommendations) • New Requirements: Carbon Monoxide Detector, ABC Fire Extinguisher, CPR / 1st Aid prior to approval • Medical & Drug Screening • New Medical Form for Other Household Members • Revised Medical Form for Caregivers • TB Test (Blood or Skin Test) • Medical Disqualification: MO endorses that person’s health condition poses a direct threat to the health and safety of others. Direct threat can’t be eliminated by modifications in policy, practice or procedures. • Ongoing Medical: Medical, cognitive or other condition that will affect child caring or just an on-going condition—requires annual evaluation. • References: New Forms • Prior Service Report • Agency and Employment Reference Forms
