DISD Student Referral Flow Chart SevereStudent has all resources needed; Parent ContactedResources ContactedAction Taken CRISIS NO CRISIS • Mild behavior Defiant, Angry, peer mediation All Students: 2 options Campus Crisis, but No immediate danger of harm Concern of immediate danger of harm (difficult to determine) Suicidal or Homicidal *Immediate danger of harming self or others Counselors Elementary: Child MH Specialist (Crisis Team as needed) Secondary: LPC (Crisis Team as needed) Call School Police Section 26 Student Needs Screening Brief Behavior Modification/ Redirection; &/or Connect with MOU/Outside Agency STUDENT OFF CAMPUS Police (city/county) can be requested to do a “Welfare Check” Need Parental Permission for Assessment STUDENT OFF CAMPUS Police (city/county) can be requested to do a “Welfare Check” Licensed Prof Counselor If no Section 26, Secondary: Licensed Professional Counselor Elementary HELP grant Child Mental Health Specialist Assist parents in seeking professional help If Behavior Intensifies: &/or Threat of harm to self/others Notify Guardian Develop Plan of Action Notify Guardian Develop Plan of Action Parents can do a Section 28; or take child to professional.
