Our Goals • Establish Shared Leadership Model • Two-way exchange with state and local public health partners and CDC leadership to enable effective decision-making • Align Goals and Resources • CDC resources effectively aligned with CDC/State goals • Improve CDC Business Practices • Reduce unnecessary administrative burden and streamline business processes associated with CDC grants

Our Role • SMOs in the States: • Senior-level coordinators placed in the SHOs Office • Perform annual assessment of CDC state-wide investments and collect/manage state/city observations regarding CDC business/program activities • PMP Atlanta • Atlanta based connectivity with CDC leadership and program staff • Coordinate PMP across all states with SMOs • Maximize information dissemination

CDC’s Extramural Investments FY 2005PMP Sites (Non-Research* Grants) • Total in PMP States: • $4,452,165,416 (2,428 grants) • 253 field staff (43% of all field staff in the U.S.) *An additional $164,284,074 is awarded to PMP states in research grants And $32,015,338 of $115,278,752 total in PHHS Prevent Block Grant Note: SMOs also now in place in NC, CO, LA
