Comparative Effectiveness Research (CER) • (CER) is the direct comparison of existing health care alternatives to prevent, diagnose, treat, and monitor a clinical condition or to improve the delivery of care. • Core question: which treatment works best, for whom, and under what circumstances.
Comparative Effectiveness Research (CER) • “The purpose of CER is to assist consumers, clinicians, purchasers, and policy makers to make informed decisions that will improve health care at both the individual and population levels." • Requires pragmatic trials • Effectiveness – benefit produced in routine practice • Efficacy – whether a benefit is produced or not (clinical trial) IOM 2010
Funding • 2010 – The Patient Protection and Affordable Care Act • Established Patient Centered Outcomes Research Institute (PCORI) • PCORI is funded through a trust fund (PCORTF) until 2019 • PCORTF will receive $320 million for 2013 • $150 million per year from congress • $1 fee per individual assessed on Medicare and private insurance plans.
Funding • $3.5 Billion from PCORTF to fund CER and patient centered outcomes research through 2019 • PCORTF will fund federal agencies, academic institutions, and private organizations • AHRQ and NIH will receive priority consideration.
Patient-Centered Outcomes Research Institute (PCORI) • Created to help manage and augment CER • Non-profit, independent organization with a multi-stakeholder Board of Governors • Sustained public-private funding for CER through 2019 • PCORI awards contracts, not grants

Funding Announcement:Assessment of Prevention, Diagnosis and Treatment Options • 3 year contracts • $500,000 total budget per year • 3rd cycle of this funding announcement • Application deadline April 15 • Will fund up to $12 million (8 contracts) • New funding announcements expected this summer
