  2. KEY DATES • November 1 - Annual Enrollment Begins! You can enroll online www.benefitenroll.com or by calling the Benefits Service Center at 877-418-0038. • November 17 - Annual Enrollment Ends! Log into www.benefitenroll.com and certify the wellness qualifiers : 1) Be a Tobacco free family 2) Complete an annual physical or biometric health screening, and 3) Complete the Cigna Health Assessment using your numeric values. Annual Physical/Biometric Screening AND Cigna Health Assessment must be completed by you and your covered spouse by 12/31. Don’t forget to make your 2014 FSA account(s) election. • November 18 – 29 Enrollment corrections can be made by calling the Benefit Service Center at 877.418.0038. Confirmation Statement will be mailed to your home. • On or about December 23– New ID cards will be mailed to home, if applicable.  If you are enrolled in the Healthcare FSA, your existing benefit card will be reload with your new pledge amount.

  3. Changes for 20142014- Wellness Qualifiers Steps Question 1:Have you, or any covered dependents, used tobacco products within the past 12 months? Yes or No Question 1a: If you say “yes” you will be asked, “Have members, who use tobacco, enrolled in or completed a cessation program in the last 12 months? Yes or No Question 2: “Do you and your spouse plan to get the required screening AND complete Cigna’s online health assessment?” Yes or No Question 2 is not required for covered children. CONFIDENTIALITY- The results of the biometric screenings, and health assessment are confidential and willnot be provided to Huber. Cigna will collect the information and inform us whether qualifiershave been met. Qualify online at www.benefitenroll.com Annual savings could be significant – as much as $420 for single coverage and $1,260 for family coverage for the Premium Option.

Changes for 20142014 Rx Plan Design Changes • Prescription Plan Changes • Slight increase in non generic copayments • Retail • Preferred brand-name drugs from $25 to $30 • Non-preferred brand-name drugs from $40 to $50 • Generic drugs copay will remain the same: $10 • Mail Order • Preferred brand-name drugs from $62.50 to $75 • Non-preferred brand-name drugs from $100 to $125 • Generic drugs copay will remain the same: $25 Express Script ID cards will be mailed in Q1. Medco cards will still be accepted.

  8. Healthcare Reform (HCR) Public Exchanges How will it impact you? • Public Exchange Notice • J.M. Huber provided an exchange notice to all employees in the annual enrollment packets. • HCR Value and Affordability Mandate • Employer group plans must offer affordable contributions (less than 9.5% of income for single coverage) and have a plan design that is greater than 60% actuarial value. • Huber’s coverage exceeds the minimum value standard (86% for Premium and 83% for Core) • Huber’s coverage meets the affordability mandates, employees earning more than $16,673* are not able to obtain a subsidy and would pay the full cost of coverage if they enroll in a state exchange plans. * Anyone earning less than $16,673 may be subsidy eligible, depending on whether their spouse is offered affordable coverage

Provided by Huber, at no cost to you Supplemental Life Insurance Options: Your choice of an additional • 1 x annual pay • 2 x annual pay • 3 x annual pay • 4 x annual pay • No additional coverage Remember: • Evidence of Insurability (EOI) is required. • The maximum amount of life insurance between Basic and Supplemental is $1,250,000. • Defaults to 2013 coverage.

  16. Flexible Spending Accounts Healthcare & Dependent FSA • Health Care FSA maximum contribution amount is capped at $2,500. • There are no changes for the contribution amount for a Dependent Care FSA it is capped at $4,550 (max company match $450).

  18. 2014 Enrollment Options • Enroll Online or by calling the Benefits Service Center at 1-877-418-0038. • ONLINE ENROLLMENT (www.benefitenroll.com) • Can be accessed 24/7. You’ll need a computer with internet access and a printer. • User ID: Year of Birth + Last 4 Digits of SSN. (example 19651234) • Passwords reset to Default as of October 31: 000 + Last 4 Digits of SSN (example: 0001234) • After first successful login, you will be prompted to change your password (must be at least 7 characters)

  19. 2014 Enrollment Options Steps to meet the Qualifiers: • Log onto www.benefitenroll.comand answer the wellness questions during annual enrollment Nov. 1-17. • Next, you and your covered spouse, complete your annual physical (or biometric screening) and obtain your numeric values needed to complete the Cigna online Health Assessment. • Lastly, complete your Cigna online Health Assessment at www.mycigna.com with your numeric values obtained by your doctor (or biometric screening) by December 31 (deadline is January 31 for new enrollees) • Your spouse will need to access their own online Health Assessment on mycigna.com. Refer to instructions flyer, “My Health Assessment”. • If you had a physical at any time in 2013, just call for your numeric values – no need to get it again. • If you do not satisfy the wellness qualifiers by the deadline, you will receive the non-wellness contribution rate in January 2014.

  21. “My Total Rewards” Statement – next statement through 12/31/2013 • Access your “My Total Rewards” Statement at www.benefitenroll.com • Your Statement features a complete view of pay and company-sponsored benefits • New features for 2013! • Statement Year Selector • Compensation History Graph • Review information whenever you need it. • Available online 24X7 from work or home • Updated two times per year. A new statement will be available on or about January 15. Be sure to check it out!

  22. 2014 Benefits – Employee Savings Plan • Annual Enrollment is a good time to review your 401(k) Savings Plan account. • Make sure you are getting the full company match (Huber matches on first 5% of your eligible pay). • Evaluate your investment allocation • Re-balance your investment, if necessary • Use easy modeling tools on website to determine if you’re on track to meet your goals.
