Outline • This presentation will provide the background, goals, implementation strategies, and early lessons learned for the recently announced Family Medicine Interest and Scholars Program supported by the BCBS of NC Foundation (BCBSNCF) and the NCAFP Foundation. • This six year $1.8 million dollar program is a collaborative effort between the BCBSNCF ($1.2 million) and the North Carolina Academy of Family Physicians Foundation ($600,000) • It is the result of two years of negotiations between these two organizations.

  4. Background • The North Carolina Academy of Family Physicians (NCAFP) has had a long term interest in medical student programs. Currently has a Student Endowment of $850,000 to support FMIGs and other medical student activities. • Senior leaders in BCBS served on NCAFP boards and facilitated meetings on the future of primary care in NC.

Background • BCBSNC Foundation has for years supported loan repayment programs through the Community Practitioner Program. • Challenge to NCAFP to make a proposal to increase student interest in family medicine specifically and primary care in general. • Two years of negotiation and development.

  6. Program Summary • This project takes a two-tiered approach to increasing interest in Family Medicine among North Carolina’s medical students. • The first involves selecting 12 Family Medicine Scholars from among the state’s four medical schools each year for this special program. • The second involves targeted efforts to increase interest in family medicine by increasing interaction between practicing family physicians and the Family Medicine Interest Groups at each of the state’s four medical schools. The ultimate goal is to increase the number of North Carolina’s medical students who enter family medicine as a career.

  7. Family Medicine Scholars Program • Selected medical student “scholars” are paired with “Master Preceptors” during the 3 years of this program. • Three broad components: • Clinical Mentoring • Leadership Training and Travel • Scholarship and Loan Assistance

Family Medicine Scholars Clinical Mentoring • Selected medical student “scholars” are paired with “Master Preceptors” during the 3 years of this program. • The relationship with these specially selected preceptors begins during a four-week externship between the student’s first and second year of medical school. This externship is supported by a stipend and housing allowance. • Scholars receive ongoing mentoring and guidance through structured meetings with their Master Preceptor throughout medical school. The students meet with these preceptors during the NCAFP’s Annual Meeting for three years, and complete additional educational experiences (such as their family medicine preceptorship or other appropriate rotation) with these individuals.

Family Medicine Scholars Leadership Training and Travel • Scholars will receive travel, housing, and participation stipends to attend in-state and national Family Medicine conferences, as well as to complete their leadership rotation. • Scholars will complete a 4 week health leadership rotation in Raleigh, NC working closely with the staff and leadership of the NCAFP • Ongoing participation in health policy activities and collaboration with their school’s FMIG leadership team.

Family Medicine Scholars Scholarship and Loan Assistance • Scholarship amount is based on location of residency training program enrollment and ultimate in-state practice. • Scholars who MATCH into a NC-based Family Medicine program will receive a $7,500 scholarship. • Scholars who MATCH into an out-of-state family medicine residency program will be provided a $2,500 scholarship. • Upon residency graduation, NC-based resident scholars who choose to practice in North Carolina will receive an additional $2,500 scholarship. • Scholars graduating from out-of-state residency programs that elect to return to North Carolina for professional practice will be awarded an additional $7,500 scholarship.

Goals of the Family Medicine Interest Group Focus • This component seeks to increase the effectiveness of relationships between: • North Carolina Academy of Family Physicians • Practicing family physicians • Medical school departments • The Family Medicine Interest Group at each medical school in the state.

Goals of the Family Medicine Interest Group Focus • This should expand the knowledge base/interest level of medical students who may be interested in family medicine/ primary care by increasing on-campus contact with practicing physicians and other thought leaders in primary care. • Increase general family physician interest in precepting

Family Medicine Interest Group • This effort includes exposing a large number of students to the NCAFP Annual Meeting each year • Practicing family physicians speak to medical students at each school on an ongoing basis, • Funding a national-level speaker at key Family Medicine Interest Groups events at each medical school.

  15. Master Preceptor Funding • Application • Stipend-$4000 • - $2000 for the summer externship • - $2000 for the second rotation as a community preceptor in the 3rd year • Annual meeting registration waivered ($380) for 3 years

Housing and participation stipend are awarded upon completion. • Scholars attend and meet with their assigned Master Preceptor during NCAFP Annual Meeting 12/2011

  17. Brody School of MedicineYear 1 MS-1/MS-2Class of 2014 • Scholars collaborate with their school’s Family Medicine Interest Group and its leadership throughout academic year. • Scholars meet with Family Medicine Faculty Advisor to review academic progress.

Brody School of MedicineYear 2 (MS-3)Class of 2014 • Scholars continue collaboration with school FMIG, assisting with programs where applicable and speak about their experiences as a Student Scholar to date. • Scholars attend and participate in North Carolina Family Medicine Day in May 2012 • Scholars attend National Family Medicine Residents and Medical Students in Kansas City 7/2012

Brody School of MedicineYear 2 (MS-3)Class of 2014 • Scholars attend and participate in various business meetings held during the NCAFP Annual Meeting in Asheville 12/2012. • Scholars complete their regularly scheduled Family Medicine clerkship as a part of their academic curriculum, with their assigned Master Preceptor. • Scholars meet with Family Medicine Faculty Advisor to review progress.

Brody School of MedicineYear 3 (MS-4)Class of 2014 • Scholars complete leadership rotation or continuity experience, including visits to the NCAFP headquarters. • Scholars attend one national Family Medicine conference • Scholars attend and participate in North Carolina Family Medicine Day in May 2013

  21. Brody School of MedicineYear 3 (MS-4)Class of 2014 • Scholars attend and participate in business meetings and meet with Master Preceptor at NCAFP Annual Meeting in Asheville, NC 12/2012 • Scholars entering a Family Medicine residency training program are awarded up to a $10,000 scholarship.

Desired Outcomes From Family Medicine Interest and Scholars Program • By June 2016, of the approximately 400 medical school graduates in North Carolina each year, an • Average of 60 graduates will enter Family Medicine Residency programs annually. This is an increase of 20 students per year, or 50%. • 1. By 2013: 45 graduates enter FM - increase of 11.25% since 2010) • 2. By 2014: 50 graduates enter FM - increase of 25% since 2010 • 3. By 2015: 55 graduates enter FM - increase of 37.5% since 2010 • 4. By 2016: 60 graduates enter FM - increase of 50% since 2010

Desired Outcomes From Family Medicine Interest and Scholars Program • By June 2016, of the 60 NC medical school graduates entering Family Medicine Residency Programs each year, 45 will remain at in-state programs (this is a goal of 75%) • By June 2016, 43 Student Scholars will choose the specialty of Family Medicine by entering FM Residency programs (48 total scholars; therefore this is 90% of all participating scholars). • We will monitor the number of scholars who stay at in-state residency programs. • By June 2019, 38 Student Scholars will have established practice in North Carolina upon completion of Residency training (this is 80% of all participating scholars)
