  2. Chronic diseases are among the leading causes of death in the United States. Chronic diseases demand extensive medical care, including frequent clinical visits, strict dietary restrictions, and routine prescriptions, among other things. RPM care management systems can collect and transmit medical data from patients to clinicians in another location in real time. As a result, doctors may monitor vital signs such as blood pressure, glucose levels, heart rate, oxygen saturation, and so on remotely. This will help patients to live more independently and improve chronic illness management.

  3. What is the relationship between RPM & CCM? RPM is an abbreviation for remote patient monitoring, and CCM is an abbreviation for chronic care management. The first, as the names suggest, is used to monitor patients' health outside of traditional clinical settings, whilst the second attempts to assist persons suffering from chronic conditions. RPM CCM can be used in tandem to help bridge care gaps and ensure that patients suffering from chronic diseases receive the treatment they require. The most obvious advantage of using RPM for chronic care is that more patients will have access to care, while costs will be reduced. RPM allows doctors to care for patients with chronic conditions remotely, with few in-person clinical visits. This provides more people with access to care.
