  1. Familial Adenomatous Polyposis – What are the Treatment Options? Introduction Familial adenomatous polyposis (FAP), is an inherited disorder characterized by the formation of numerous adenomatous polyps in the large intestine or colon. These polyps start out as benign, but if left untreated, they can transform into colon or colorectal cancer. There are three different types of FAP, but all of them are caused by genetic defects or mutations. One is the •ClassicFAP, which is the most severe and most common,

  2. •the second attenuated FAP, •And the third one is autosomal recessive FAP. The growth of polyps starts as early as the teenage years. The average age when these polyps turn malignant is when an individual turns 39. The growth is delayed in attenuated FAP, but malignancy occurs around 55 years of age. People with the autosomal recessive type of this disorder have fewer polyps than those with the classic type. Treatment In case of a few small polyps, doctors can remove them during a colonoscopy exam. Eventually, though, polyps may become too numerous to remove individually. To prevent cancer, surgery is recommended for familial adenomatous polyposis, usually by late teens or early 20s. Surgery may not be required for attenuated FAP. Most colorectal surgery is done using minimally invasive (laparoscopy) techniques. Laparoscopic surgery is performed through several small incisions that require just a stitch or two to close. There are 3 types of surgeries which are used to FAP and related complications, these include: •Ileal pouch-anal anastomosis surgery or J-pouch surgery: The colon and rectum are removed in this surgery, while preserving your anus, allowing you to have normal bowel movements. •Total Colectomy: The colon is removed while preserving your rectum and anus, allowing you to have normal bowel movements. •Continent Ileostomy: In this procedure, your small intestine is connected to the outside body through an opening (stoma) on your abdomen. Bowel movements that would normally have emptied through the rectum are collected in a waste bag that attaches at the stoma.
