  4. KNOWSLEY YOUNG CARERS PROJECT • Non Statutory Service. • Based in centre of borough. • Supported by a charity. (Friends of Knowsley Young Carers).

  5. TARGETED SUPPORT FOR YOUNG CARERS • Across the UK young carers projects have increased from 110 in 1999 to 350 current projects working with around 25,000 young carers. • Young carers responding to the consultation made it clear that better support for their family and the person cared for was priority. Carers at the heart of 21st century families and communities. Govt. report

  6. WHY WORK WITH THE WHOLE FAMILY? • In a system that thinks family both adults and children's services join up around the needs of the family. Services would: • Have no ‘wrong door’ • Look at the whole family • Build on family strengths • Provide support tailored to need Think Family: Improving the life chances of families at risk 2008. Cabinet Office.

  7. BUILD UPON STRENGTHS – RAISE ASPIRATION AND RESILIENCE. • Assessments and plans. • Groupwork with Parents. • Activities with Families. • Community Involvement. • Strategic Involvements.

  8. KNOWSLEY YOUNG CARERS PROJECT THE ROLE OF THE SPECIALIST NURSE

  9. THE NURSE • IS The primary nurse for those who have left school • IS The primary nurse for those not on school roll • IS NOT the primary nurse for those in school • IS NOT the primary nurse for those on school roll even if not attending

  10. CHILD NEW ONTO PROJECT • School nurse informed • Health advisor consent form obtained • Child medical health records accessed • Health assessment commenced • Outstanding health needs identified • Health action plan • Provide information and support around health condition of cared for person

  11. CHILD ON PROJECT • 6 monthly review • Invite to health promotion sessions • Health advice for parent/carer/family • Individual advice and support • Liaise with school nurse

  12. OTHER RESPONSIBILITIES • A health resource for project staff • Inform other professionals about project • Encourage referrals • Regularly work in Safeguarding Children's team (Health team) • Represent young carers at a strategic level
