ETT No. ,depth IV fluid: strength, rate, infusion pump Monitor : SpO2, HR, color 6.บอกให้ RN ต้นทางคุยกับRN ที่ ICU (NICU 089-901-9652, PICU 089-901-9653) รพช. ศูนย์ Refer ประสาน 1.แจ้ง RNรพช. โทรส่งรายละเอียด ให้ ICU 2.โทรแจ้ง Intern ที่ ER (ข้อมูล) Staff โทรบอก Intern ER – check ETT, HR, IV fluid RN ICU - problem, อุปกรณ์ที่ต้องเตรียม Intern กุมาร2-3 • RN ER • ส่งญาติทำ admission • โทรแจ้ง staff กุมารทันทีถ้าผู้ป่วยดูอาการไม่ดี • คลำ pulses หรือฟัง heart rate • บันทึก ETT no, depth , IV rate • -โทรส่งเวร RN ICU • -ถ้าผู้ป่วยตัวแดงดีส่งเด็ก admit ได้ทันที • (กรณีผู้ป่วย PICU ให้วัด vital sign เหมือนกรณีผู้ใหญ่) Intern ER check patient - ETT No., depth - IV rate, strength+ infusion pump - Keep warm: ถุงพลาสติก ถุงถั่วเขียว transport incubator ER ICU RN ICU เมื่อได้รับโทรศัพท์จาก RN ER จะโทรศัพท์บอก staff กุมาร และ intern กุมารทันที ประชุม PEER REVIEW จาก PCT กุมาร 9 กุมภาพันธ์ 2554
