The Evolution of CEED in LA • First round • 2 funded legacy projects • TUSPHTM: Interactive DVD Project • LCCP/ACS: COFFEE Program, Policy Support and Hospital In-Reach • Second round • LA CAP funded • COFFEE and In-Reach (supplemented by DVD) and Policy • Third round • Focus on COFFEE program • Leveraging other resources to fund policy and In-Reach

  4. The COFFEE Program • 35 community network partners • 5 unique communities • 30 COFFEE-Mates trained • Educating women on breast & cervical screening • Navigating women to screening services

Successes • Leveraging resources to grow COFFEE • Community Network partner received Avon funding to expand program to Jefferson Parish • LBCHP received additional funding to continue including Latina and Vietnamese communities • ACS received funding for home health parties from LSU School of Medicine MB-CCOP

Successes • “Abreast a Boat of Hope” DVD disseminated • Recipients included: • Disaster Navigation Program Community Advisory Board • Franklin Avenue Baptist Church • Watson Memorial Church • American Cancer Society COFFEE program • McFarland Institute • Orleans & Jefferson Parish Community Neighborhood health centers

Successes • Moving some CAP activities to other funding sources • The In-Reach program was funded by NBCCEDP supplemental funds • Louisiana Comprehensive Cancer Control Program received a CDC policy grant to support policy work • Sustainability • “Abreast a Boat of Hope” DVD was distributed to COFFEE program and In-Reach program to enhance educational outreach activities
