Co-production 800 Providers access to equipment could improve outcomes for clients more effectively Providers should be able to flex levels of support without the approval of case managers Extend services to be available 24 / 7 Availability of information, advice and guidance Providers and voluntary organisations could carry out assessments and reviews for KCC Increase the amount of extra care housing Professional staff should not spend too much of their time on administration and data input Avoid discharge from hospital straight from hospital to residential care KCC needs to be better at commissioning both in-house and external provision Need for KCC to trust others and work in partnership with providers and other organisations Let’s use technology to best effect Provide brokerage so that people can do proper support planning Encourage people to do more for themselves – where they are able Don’t always buy what looks cheapest in the short term – base purchasing decisions on the true business case More support for carers Case managers are hard to get hold of to speak to – need contacts that are available Need to work more closely with health and housing providers Enablement should be an ethos for all home care and residential care
