State ReportsDOH Annual School Report • August Standard Release Inclusions • Changes to State Requirements: • Varicella • Added Grade 2 • Kindergarten students must have 2 doses given on or after the 1st birthday (4 day grace period applies). • 1st , 2nd and 6th grade students only need 1 dose of vaccine. • Varicella to be broken out into Dose #1 and Dose #2 on the DOH State Report for entry level students. • Parent Reported history not acceptable, need physician note.

State ReportsDOH Annual School Report • August Standard Release Inclusions • Changes to State Requirements: • HepB - Add Alternate Schedule for HepB (10 micrograms) • Minimum age – 4 days before 11th birthday. • Minimal interval between Dose #1 and Dose #2 – 4 days less than 4 lunar months (108 days). • Maximum Age – 4 days after 15th birthday. • Students who don’t get Dose #2 before 15th birthday + 4 days would need to complete series with 2 more doses of regular HepB (5 micrograms)

State ReportsDOH Annual School Report • August Standard Release Inclusions • Changes to State Requirements : • HepB • A data manipulation has been run to add a new vaccination code HP2 to the required vaccination table • Added Grade 11 • DTaP/DT/Td/Tdap • Added Grade 7 • 6th grade students who are 11 or older whose most recent vaccination is DT (no Pertussis) will show on the Out of Compliance and At Risk reports as Report Generated Conditional (RGC)

Hierarchy is as follows: • Medical • Personal • Religious • Example: Student has a Personal waiver type on their VAR vaccination and a Medical waiver type on their MMR vaccination. The student will be reported under the Medical Exempt box in Section 2 of the Pre-School Immunization Status Report.
