  2. 事故概況 2009年10月23日(星期五)凌晨12:23，波多黎各由海灣石油經營的Caribbean Petroleum Corp. 發生大爆炸，火災延燒兩天，造成附近民房及經過的車輛玻璃破裂，計有兩人受傷、1000多人疏散，油槽區40座油槽計21座燒燬，槽內儲放的產品有汽油、柴油、航空燃油及燃料油(該公司煉油廠已於2000年關閉)。 經FBI調查排除為人為縱火或恐怖攻擊，警察出動直昇機勘災、消防單位積極在周遭噴水降溫，由於煙霧過大當局下令封鎖該區的空中交通，居民則被告知關閉門窗居家避難。

  9. 事故原因 • 經美國CSB五人小組調查，初步認為係港口油輪卸油至汽油槽時油槽發生溢流，且未被偵測到，致汽油蒸氣溢散達直徑2,000呎，因西北側的蒸氣雲碰觸到火源，導致火災爆炸。 • 造成油槽溢流的原因： 1.監控油槽液位之電腦化系統未操作使用 2.操作人員係利用油槽外壁的機械式液位計 量測液位，凌晨卸油期間控制室內的人員 未查覺發生溢油情形。
