  3. LOTO hardware • The lock – only 1 key • The tag – employee-specific • The locking device

  4. Hazardous Energy Any type of energy is potentially hazardous! Common forms of energy: • Electrical • Gravitational • Hydraulic • Kinetic - residual movement in rotating parts

  5. Importance of LOTO • Anyone working on machinery may be exposed to injuries from energy surges, machine startup, or release of energy that is stored in the equipment • In metal fabrication: • 10% of fatalities due to failure to use LOTO procedures • 15% of serious trauma cases are LOTO-related

  6. Authorized? “Authorized” employee: • Specially trained in LOTO procedures • Allowed to lockout a piece of equipment • Allowed to restart a locked out machine • Has a LOTO lock with only 1 key • Has a LOTO tag with their picture & name on it

  7. Affected? “Affected” employee: • No LOTO responsibilities • Must recognize when LOTO is in progress • Not allowed to remove locks and tags • Not allowed to bypass locks and tags YOU will be an “affected” employee unless trained in LOTO

  8. Training • Required for “authorized” and “affected” employees • Must be provided by the employer • Re-training must be provided when there is a change in: • Job assignment • Machinery or processes - a new hazard is present • Energy-control or LOTO procedures

  9. LOTO Shut-down You - the “affected” employee - will be notified that: • LOTO is about to occur • You cannot use that machine until the person who locked it says it is safe to do so

  10. Don’t Use a Machine • From the time you are told a machine is being shut down until you are told it is safe to use • If you see a lock or a tag: • Do NOT bypass it • Do not try to remove it

  11. When in Doubt: ASK! A life is on the line!

  12. Lockable Disconnects • In plain view from your workstation • One for each energy source • If you notice that a disconnect was missed during machine lockout – speak up!

  13. LOTO: Start-up You - the “affected” employee - will be notified that: • Equipment will be restarted – stay away! • All LOTO devices have been removed The “authorized” employee must test the equipment to be sure it is safe, before YOU can use it.
