Connected Communities Update –Suffolk/East Suffolk Context • TCA Localism strand – ambition to integrate community development resource across the ‘system’ = more coherent offer to communities and less duplication [ENABLING] • MASH review – ‘deep dive’ in Waveney. Gaps around early help for individuals and families who don’t meet thresholds, re-referrals and feedback [EARLY HELP] • Enabling Communities Strategy approved by SCDC and WDC in November. Delivery Plan identifies who and what the ‘enablers’ for each of the 12 stages of the ‘enabling’ model are [BOTH] • CCG Communities Delivery Plan agreed by NHS Great Yarmouth and Waveney CCG. Priorities discussed and agreed with Ipswich and East Suffolk CCG [BOTH]

Connected Communities Update – Enabling • Implementation of the Enabling Communities Strategy and Delivery Plan – joint ‘hands-on’ training early in 2016 for all community development staff • ESP outcome proposal to fill the gap around in-depth business/project planning support for communities (OP1) • Further development of the TCA integration project e.g. location, staff in scope, priority outcomes to be delivered • Input into delivery of Suffolk Volunteering Strategy (led by CAS and approved by HWBB at last meeting), including innovative approaches (e.g. Timebanking), more business/corporate volunteering and enhanced youth volunteering offer • Support complementary activity such as the CAS ‘Grow Your Communities’ campaign • Enable more Dementia Action Alliances, Dementia Champions and Dementia Friendly Communities

Connected Communities Update –Early Help • Five Community Health workshops – dementia, mental health, carers, social isolation and being active – to be held January to April in Waveney and March to June in Suffolk Coastal • SCC Public Health funding to support emerging community projects through a themed ‘community call to action’ • Local Area Coordination – outcome proposals developed for ESP and Better Care Fund. Proposes 2 LACs in East Suffolk to provide early person-centred support to individuals and families, with services as a ‘last resort’ (OP2) • Synergy with other community-based early help initiatives, including Good Neighbours (CAS) and Men’s Sheds (Halesworth, Leiston and Pathways Care Farm) • Social prescribing pilot planned for Waveney (through Better Care Programme Office) • Greater use of ‘bridging’, ‘peer’ and ‘volunteer’ health roles e.g. Kirkley Navigators, Trusted Advisers, Health Coaches
