  3. Indicaties • Dikke darmkanker • Diverticulose • Ziekte van Crohn, ColitisUlcerosa

  4. Incisies

  5. - Medianelaparotomie (bovenbuik/onderbuik/xiphoid-pubis) • - Dwarselaparotomie • - Subcostalelaparotomie (evtbdz) • - Wisselsnede • - Minilaparotomie • - Pfannenstiehl • - Transplantatieincisie • Thoracoabdominalebenadering • LAPAROSCOPIE

  7. Colectomieen: Hemicolectomie re Ext. Hemi-R Transversumresectie Hemicolectomieli Ext. Hemi-L Sigmoidresectie

  8. Rectum operaties • Abdominoperinealeresectie, sluitspier wordt weggenomen, colostoma • Low anteriorresectie, anastomose met/zonder beschermend ileostoma • Hartmannresectie, geen anastomose, colostoma

  9. Stoma’s • Dikke darmstoma, linker onderbuik • Eindstandigcolostoma • Dubbelloops colostoma • Dunne darm stoma, rechter onderbuik • Eindstandigileostoma • Dubbelloops ileostoma

  10. EindstandigColostoma • Rectumcarcinoom < 5 cm van de anus, APR • Oude patienten, anastomose te risicovol • Acuut, ileus, perforatie

  11. Dubbelloops colostoma • Rectumcarcinoom, pre-operatieve radiotherapie • Ontlastend, soms tijdelijk • Oude patient waarbij resectie niet mogelijk is

  12. Eindstandigileostoma • Na subtotale colectomie bij CU of meerdere poliepen

  13. Dubbelloops ileostoma • Bijna altijd tijdelijk ter bescherming van anastomose bij LAR bij rectumcarcinoom na rt of bij bescherming van een ileoanale pouch • Opheffen na 3 mnd

  14. Colostoma • Helaas ook bij re-laparotomie bij naadlekkage na eerdere operatie • Of bij gynaecologische of urologische operatie
