Denominator= Column R • The total number of children enrolled at any point in time and still enrolled in the home visiting program who were referred to community services before the last day of the previous month.  • Numerator= Column S • Enter the total number of children enrolled at any point in time and still enrolled in the home visiting program who were referred to community services before the last day of the previous month and received community services within 30 days. • Who is included? Any child referred to a community resource for whom there was a concern • For how long are people included? This is a cohort measure- For Aug data- a child who was referred to a community service at any point before and including July 31, 2019 remains in this count until they dropout or graduate. The child belongs in the numerator if the child received the community service within 30 days. • A concern was identified for Gabriela in June. She was referred to a Mommy & Me group on June 17th. Gabriela’s mother and Gabriela went to the Mommy & Me group on July 27th. For reporting July data, a child who was referred to a EI at any point prior to and including June 15, 2019 remains in this measure until they dropout or graduate. • A concern was identified for Tony in May. He was referred to EI on June 7th. Tony received his IFSP planning meeting on July 7th. Tony left your program on August 15th. Denominator= Column AC • The total number of children enrolled at any point in time and still enrolled with parent concerns, Home Visitor concerns and/or a positive screen who were referred to the appropriate combination of services. • Numerator= Column AD • The total number of children with identified developmental or behavioral concerns who received all of the services that were determined to be appropriate for her/him within the expected timeframe. • Who is included? • Any child for whom there was ever a concern, who is still enrolled who was referred to the appropriate combination of services. • For how long are they included? • This is a cohort measure. When reporting for Aug, you would include all children for whom there was a concern and referral to at least one of the following: HV promotion, community services and/or EI. • In early June, a concern is identified for Eddie. Eddie gets home visitor promotion by the next home visit. Eddie is also referred to EI on June 5th.
