  4. Services Provided The Nutrition Expert • Supermarket tours • Individual nutrition counseling • Educational programs: • Diabetes • Cardiovascular disease, • Celiac disease • Bone health • Sports Nutrition • Individual nutritional counseling

  6. Overtime…. • Individual Consultations grew into MNT – driven by physician referrals • Demand for services & requests grew

  7. What IF???? • Focusing on our Mission Statement – Making Lives easier, healthier, happier. Lives = One Customer, One Community One EMPLOYEE at a time. • Could we change the lives of our own employees? • Could we lower our own healthcare cost?

Things to ask… • Is the employer group a self funded insurance plan…??? • Is it more costly to bill insurance company directly or pay out of pocket? • What types of services will the benefit program cover??? • MNT??? • Standardized education programs???? • What are the costs of these services, don’t give them away!! • What is the desired outcome???

Where do we focus??? • 2008 – 14% report using tobacco • 2009 – 16% report using tobacco • 2008 – 46% HTN • 2009 – 43% HTN • 2008 – 23% high cholesterol • 2009 – 16% high cholesterol • 2008 – 18% elevated Glucose • 2009 - 14% elevated Glucose • 2008 – 34% overweight or obese • 2009 – 34% overweight or obese Metabolic Syndrome

  10. Started in 2008….. • Incentive structure program…. The more they participate the more they save. to engage employees in improving their health…. What would motivate employees???? $$$$$$$$$$$$$$$$$$$$$$$$$

Results • 2009 • 7,730 employees completed all requirements • 9,413 Personal Health Assessments completed • 2010 • 10,633 employees completed all requirements • 10,801 Personal Health Assessments completed • 11,844 biometric screenings completed at our stores • 2011 • 10,558 completed all requirements • 12,054 PHAs completed • 12,493 biometric screenings completed at stores

  14. 2012 Results? • 11,256 employees completed all requirements – 80%! • Over 18,000 biometric screenings were completed at Hy-Vee stores • Over 450 employees completed the Begin program

  16. Results Statistics The following graphs represent the percentage of Begin participants who have eliminated the given risk factor for metabolic syndrome and the percentage of those who have not.
