INCIDENT 0603 hrs approximately 18 kilometres from PDO Marmul a saloon car Travelling towards Marmul veered across the road into the path of an Oncoming Vehicle travelling towards Nimr, the result was a head on collision. The Passenger Was killed on the spot while the driver succumbed to his injuries at the PDO Marmul Clinic. The Driver of the pick-up was able to get out unassisted and only suffered minor knocks and bruises. ROOT CAUSES: Lack of care and attention by a 3rd party driver while driving caused by possible Fatigue and poor visibility Lateral Learning Ensure Journey Plan considers driving during daylight. Driver to comply to the Journey Management Plan Recommended Actions: Discuss this alert in toolbox talks and HSE meetings and Distribute to all contractors Post on HSE notice boards Include in site HSE induction • Key Learning: • Seat Belts do Save Lives • Avoid driving in poor visibility Head On Collision: Double Fatality Contact: MSE511 for further information or visit HSE website Alert No 47 March, 2011
