You can search by one of the following provider types: Frontline/Industrial Medical Clinics Ambulatory Surgery Centers General Hospitals Other Facilities & Services Primary Treating Physicians Other Physicians & Healthcare professionals Select a provider type from the drop down menu Searching by Provider type…
To produce a list of providers within a specific mileage enter the address and # of miles from location you wish to search To create a list of providers, Click on the ‘Find Provider Now’ button Note: if you wish to ‘drill down’ to within a specialty to find a provider by language, or board certification, select ‘further refine search’.
