Child Well-being Operational Guidance Common Assessment Framework Process A If at risk of significant harm go straight to Safeguarding Children Procedures Child / Young person identified as having unmet need Process start Step in process Check who’s working with child / young person already, use CWB system as appropriate Decision Process End CWB – Child Well-Being LP – Lead Professional CAF – Common Assessment Framework Is there a CAF / LP already completed ? Gain consent from family contact LP or practitioner undertaking the CAF and discuss needs Yes No Determine whether a CAF needs to be completed. Use Pre- Assessment Checklist as appropriate Is it appropriate to complete a CAF ? Follow Procedure B Yes No Can the child’s needs be met by a single agency ? Yes Continue support or make referral using agency process Use Family Services Directory to identify Appropriate services No

No Devise action plan and continue to work together to deliver the actions from the plan Input information onto CWB system Yes Arrange a CWB meeting with relevant services Devise an action plan, identify a LP and set a review date Input information onto CWB system End provision or continue to sustain and improve outcomes Close package on CWB system Yes Are needs being met ?
