Professor Archibald Leman Cochrane, CBE FRCP FFCM, (1909 - 1988) • Archie Cochrane是當時英國內科醫師及著名的流行病學專家，他對於流行病學發展的偉大貢獻，以致 Cochrane Collaboration 以他的名字命名以表達對他的敬意 • 1972年出版的著作- Effectiveness and Efficiency: Random Reflections on Health Services 明確的提出以下重要論述 • 由於資源終將有限，因此應該使用已被證明有明顯效果的醫療保健措施 • 來自於隨機對照試驗（Randomised Controlled Trials, RCTs）的證據，比其他任何證據更為可靠 CBE 司令勛章（不列顛帝國勳章第三級) FRCP 皇家內科醫師學會會員 FFCM 皇家內科醫學院公共生醫學科院士

  5. 實證醫學發展 • 1979 -> Archie Cochrane 教授倡導應依據特定的疾病及治療方法，蒐集並分析所有相關的隨機對照試驗，同時隨著新資料的出現不斷更新，以便獲得更為可靠的結論 • 1980 -> 加拿大著名臨床流行病學家 David Sackett 教授將臨床流行病學的方法和原理實際應用於臨床指導，以提高臨床療效，為實證醫學的方法學奠定重要基礎 • 1992 -> 全世界第一個 Cochrane中心在 National Health Service (NHS)的資助下於牛津大學成立，旨在促進及協調醫療保健方面隨機對照試驗系統評價的生產和保存，以便依據最好的科學進展和研究結果服務於臨床醫療、衛生管理和高層決策 • 1993 -> Cochrane Collaboration正式成立

  6. 實證醫學五項步驟 • 步驟一: 將臨床上遇到的問題轉換成一個可以回答的問題 Patient‧Intervention‧Comparison‧Outcome • 步驟二: 尋找可以回答這個問題的最佳實證資料及相關文獻 原始文獻 v.s. 評論資料 • 步驟三: 謹慎評估這些資料對於臨床的正確性、作用及適用性 • 步驟四: 將實證資料與臨床專業及病人特殊狀況整合，並找出最適當的臨床決策 • 步驟五: 評估以上的步驟及治療效果

  7. 為什麼我們需要 The Cochrane Library？ 節省時間 臨床人員每天必需接收各式各樣的醫學相關資料，所接觸的訊息來自專業醫學研究報告、學會研究發展訊息、政府部門政策消息或藥廠發佈的藥品研發資料 全世界每年有超過 2,000,000 篇論文發表於 20,000 種生物醫學專業期刊，這些文章的累積高度相當於一棟台北 101大樓 (500公尺)！每天至少需要閱讀19篇論文才能掌握核心發展的最新狀況

  8. 醫學界重要的出版品 The Lancet、New England Journal of Medicine、British Medical Journal and the Journal of the American Medical Association 一致推崇 Cochrane Reviews 是目前最具參考價值(“gold standard” )的系統性評論(Systematic Reviews) ，同樣的評價也出現在其他不同領域地醫學專業期刊中

Cochrane Database of Systematic Reviews (CDSR; Cochrane Reviews) • 針對特定的疾病或其他健康照顧方面的問題給予系統評論定義，並裁定這項治療方法是否有效 （52個系統評論小組） • 孕婦生產過程有親友的陪同，是否有正面的幫助 • 戒菸與心臟病突發的機率 • 編輯群為了做出總結而盡其所能的尋找、評估及綜合來自各種相關的、尚未出版的及非英文語系的科學研究資料。他們提供治療效果的總體結論，同時將每一項結果以一個獨特的標題標示，因此使用者可以輕易的查詢相關的原始文獻 • 系統評論與其他評論不同之處，在於他們堅持一套嚴格的標準，使研究的納入，資料之提取、分析和整合，更具品質，以降低誤差的機率，並確保所獲得的結果更具可信度 • Impact Factor 2007 = 4.654 (14/100 Medicine, General & Internal) • 包含 3,737筆 Reviews及1,939筆進行中的 Protocols

Database of Abstracts of Reviews of Effects (DARE; Other Reviews) • 收錄發表於醫學期刊、書目資料庫等科學文獻中的評論性文章的摘要 • DARE是唯一收錄經過嚴格標準審核的系統評論摘要資料庫；每一筆摘要包括總結概述，以及審核標準的說明 • 內容廣泛地涵蓋與健康相關的治療方法及各種領域的摘要內容，包括診斷測試、公共衛生、健康促進、藥理學、外科醫學、心理學等 • 內容由設立於英國約克大學（University of York）的 Centre for Reviews and Dissemination (CRD) 所提供 • 收錄 9,403筆資料

Cochrane Central Register of Controlled Trials (CENTRAL; Clinical Trials) • 資料來自MEDLINE及EMBASE書目資料庫，數百種專業醫學期刊、會議資料及其他尚未出版的臨床實驗計劃；內容包括文章標題及出處，多數亦含摘要內容 (不包含全文資料) Effect of low energy diet and weight loss on major risk factors, central obesity and associated disturbances in patients with essential hypertension. 低熱量飲食對原發性高血壓之控制成效 • 大約有五分之三的內容來自MEDLINE。每個系統評論小組負責維護及更新資料內容，並由US Cochrane Center進行彙編及校對的工作。 • 全世界收錄最多且最大的隨機臨床實驗資料庫 • 收錄超過56萬筆資料

Cochrane Methodology Register (CMR; Methods Studies) • 蒐集針對對照實驗的方法產生之出版品書目資訊 After the Women’s Health Initiative: decision making and trust of women taking hormone therapy.婦女健康促進計劃研究與賀爾蒙替代療法 目的: 評估 WHI 報導後之療法停用、反應、對醫學建議之信任感及未來預防行為 (電話調查) • 資料來源包括期刊文獻、圖書和會議記錄；這些文獻是從MEDLINE和人工查找取得 • 內容包括文獻標題、出處，且部份案例包含有文獻摘要 (不包含全文資料) • 收錄11,257筆資料

Health Technology Assessment Database (HTA; Technology Assessments) • 提供健康醫療技術之評估，包括探討治療之醫學、社會、道德、經濟等方面的研究 Accommodative intraocular lenses for age-related cataracts. 目的: 調節型人工晶體 vs. 老年白內障術後之近物觀看 • 目的在於改善健康照顧的品質及成本效益 • 內容由設立於英國約克大學（University of York）的 Centre for Reviews and Dissemination (CRD) 所提供，參考資料來源包括INAHTA及其他健康科技評論組織 • 收錄 7,591筆資料

NHS Economic Evaluation Database (NHSEED; Economic Evaluations) • 收錄保健醫療之經濟性評估的文獻摘要。所謂經濟性評估，意指cost-benefit、cost-effectiveness或cost-utility的分析。文獻主要來自重要醫學期刊及書目資料庫 Achalasia treatment in the elderly: is botulinum toxin injection the best option? 目的: cost-effectiveness analysis of BTI in comparison with ED • 收錄治療方式的比較、成本和治療效果等相關資料，內容同時討論文獻的品質並強調優缺點 • 收錄超過5,000篇與成本效益評估有相關的摘要資料 • 內容由設立於英國約克大學（University of York）的 Centre for Reviews and Dissemination (CRD) 所提供 • 收錄超過 24,000筆資料
