  2. Pre-contemplation • Person is not considering change in their problem behavior. • What is plan of care? Teaching plan?

  3. Contemplation • Person considers that they have a problem and the feasibility and costs of changing that behavior, but are taking no steps to change. • What is plan of care? Teaching plan?

  4. Preparation • Person has made a decision to change but has yet to act on it. • What is plan of care? Teaching plan?

  5. Action • Person is actively taking steps to change, and normally continues for 3-6 months • What is plan of care? Teaching plan?

  6. Maintenance/Relapse Prevention • Person is trying to maintain a current level of change by preventing relapse. • What is plan of care? Teaching plan?

  7. Relapse • Failure to continue action or maintenance stages begins another cycle. • What is plan of care? Teaching plan?
