  4. Klinisches Bild • Schmerz, Hämatom • drop arm sign • Jobe Test pos • Kraftabschwächung

  5. Diagnostik • Klinische Untersuchung !! • Sonographie • MRI ? • Arthro MRI !

  6. OP Indikation

  7. Risseinteilung • halbmondförmig (crescent) • u förmig (u shaped , retracted )

  8. Spezialinstrumente und Ankersysteme • Dübel staple schraub-systeme • suretac • bioheaded corcscrew • Nahtsysteme • mitec anker • corcscrew

  9. Technik • Anästhesie:ITN, ev reg. Blockade (Winnie) • Lagerung: Beachchair, Seitenlage • Zugänge: Standard posterior, anterior, lateral, Neviaser • Instrumente • Naht und oder Dübel

  10. Vorteile • kleine Zugänge (= k(l)eine Narben) • Inspektion und Therapie Glenohumerales Gelenk • keine Ablösung m deltoideus • geringerer Weichteilschaden • geringere postop Schmerzen und Rehabilitation • Geringeres Infektrisiko

  11. Nachteil • technisch höchst anspruchsvoll • Umstieg auf offene Verfahren muß beherrscht werden(Aufklärung)
