Company Profile TRIMEC Engineering and Ltd was established in Ukraine in 2006 as a partner of MAK&EL Engineering to serve on mechanical and electrical design and installation in construction field. Our experience and power comes from our partner company MAK&EL Engineering which has managed to complete many projects in the same field since 1996 in countries in Russia Federation and Turkey. Our main principle is to find the optimum solution by minimum cost not ignoring the engineering facts at the same time by our high educated and experienced staff who has participated in very international projects. TRIMEC Engineering and Trade Ltd. Ukraine is growing very fast with MAK&EL Engineering in Russia Federation by management method which is based on considering her employees as the partner of the company. TRIMEC is a very professional and modern company with its centralized staff of more than 10 engineers and 50 technical personnel.

HEALTH, SAFETY,ENVIRONMENTAL POLICY Health, Safety, and Environmental Protection regulations are TRIMEC’s first level prioritised concerns in undertaken projects. In order to secure compliance with HSE regulations; • From bid preparation to final acceptance and hand over stages of the project to the Employer, TRIMEC makes risk analysis for all work activities considering Health, Safety, and Environmental impacts. • TRIMEC makes early planning and takes necessary precautions prior to commencement of site activities in order to minimize HSE risks and impacts to acceptable levels. • TRIMEC provides involvement of employees in HSE regulations and policies. SAFETY PLAN SAFETY POLICY TRIMEC undertakes construction works by planning in advance all safety measures and assures that the working methods comply with the best industry practices relating to health, safety and environment protection and welfare of the employees of both itself and others. During the execution of the works utmost attention is paid to compliance with all statutory provisions and regulations in the country where the works is to be executed in force relating to health, safety, environmental protection and security. To accomplish the above premises an adequate Safety Plan is issued in advance to be implemented by TRIMEC's site management team upon approval of the Company. The site management team will implement the Safety Plan diligently and shall be responsible for ensuring that all personnel at the work site comply with the requirements of the Safety Plan. All employees shall receive safety training and an adequate safety information shall be distributed to the field supervisors and workers for prompt reference for the application of required safety measures.

    15. TRIMEC Engineering & trade ltd.co. SAFETY PLAN OBJECTIVE OF THE PROGRAMME The Project Safety Program has a number of specific objectives: 1. To ensure that all personnel working at the site both understand and comply with Safety Procedures. 2. To implement a safety training program to highlight known hazards and the procedure to be followed to minimize or eliminate such hazards. 3. To establish a reporting system for all accidents and dangerous occurrences, together with a procedure for full investigation and follow-up.

HOME OFFICE SAFETY ORGANISATION TRIMEC's home office organization for safety and health protection during project will include a safety coordinator who will be responsible of preparation of adequate cooperate standard procedures. Safety manuals reporting forms and other requirements to be used as reference documents for the preparation of the project safety programs to be approved by the site manager. He will also be responsible for the coordination of safety personnel to be assigned to the construction site and for ensuring adequate and timely back-up relevant to safety matters including preparation and processing of all documents involved.

WORK SITE SAFETY ORGANISATION A fully dedicated work site safety officer will be assigned to the site team to ensure the implementation of the safety program prepared in advance. The field safety officer shall report to site management. Site manager will monitor safety matters during construction work. Responsibilities of the safety officer will include: 1. Regular inspection of the work site, in association with line supervision, to ensure that only safe methods of work are applied and that all safety requirements including statutory regulations are being observed. The safety officer will take action to rectify faults in accordance with the Safety Plan and Procedures: 2. Investigations to determine the causes of accidents and dangerous occurrences and recommending measures to prevent recurrence of incidents; 3. Consultation to and discussions with the personnel and advising them concerning safe working practices, injuries or dangerous occurrences; 4. Keeping an up to date record of all information in connection with accident prevention, including accident/injury reports and analysis of accident trends; 5. Advising on and assisting with the safety training of all levels of the workface.

    18. TRIMEC Engineering & trade ltd.co. SAFETY PLAN 4. SAFETY PROCEDURES 4.1. ACCIDENT REPORTING SYSTEM 4.1.1. OBJECTIVES The primary objectives of accident reporting and investigations are to: a) establish the causes of accidents so that action can be taken to prevent a recurrence; b) establish a systematic routine method of reporting all details of circumstances immediately following an incident; c) complete an accurately documented record of each reported accident to which reference may be made at any subsequent inquiry, prosecution, or common law action. 4.1.2. INCIDENTS TO BE INVESTIGATED All accidents that result in property damage or personal injury must be investigated. The "near accident" which does not necessarily result in injury or damage, is equally important from the point of accident prevention and therefore should also be thoroughly investigated.

    19. TRIMEC Engineering & trade ltd.co. SAFETY PLAN 4.1.3. ACCIDENT INVESTIGATION Accident Investigation must be conducted in a manner which will provide facts rather than faults. a. Responsibilities i. Supervisor/Foreman : should carry immediate investigation of each accident within his area of responsibility and submit the accident report to his supervisor, to Safety Officer and keep one copy for himself ii. Safety Officer : should verify the report of supervisor / foreman and carry out independent investigation within 24 hours. iii. Site Manager : should review all reports and ensure that corrective action has been taken and observe any trends that may require action.

Questioning the man in charge and finding out which unplanned events took place over the planned events. ii. Finding out the injured man's job or the normal position and function of the damaged equipment’s or plant. iii. Questioning the injured man as soon as possible. iv. Questioning the witnesses. v. Studying the environment and examining the equipment and plant for signs of misuse. vi. Finding the explanation for any irregularity, vii. From the information obtained, establishing the reason why unplanned took over from viii. Recommended methods of implementing corrective action.

    21. TRIMEC Engineering & trade ltd.co. SAFETY PLAN 4.1.4. INCIDENT REPORTING In case of incident the following reports will be produced. a. Verbal Report: As soon as an incident is noticed in an area, it will be informed by the response of the crew in that area, verbally to his superior and the Safety Officer in case of any of the following: - casualty - serious injury property damage - equipment damage - fire - potential risk areas b. Written Report: The responsible area supervisor/foreman and Safety Officer will issue to Site Management a written report of any incident within 24 hours from the event. The report for serious cases will include written statements, photographs and detailed description of the accident.

Monthly Report of Incidents: At the end of month the Safety Officer will issue to Site Management a report detailing all the incidents occurred in the month. By the 10th day of each month MTG will issue a safety, health and fire damage report to Company. 4.2. SAFETY INSPECTIONS Adequate planned and random inspections will be regularly performed by Site Management safety advisors/engineers and senior management personnel, to verify the correct implementation of all safety practices required by the safety program. These inspections will be performed on the basis of established check lists, prepared in accordance with the facility/activity to be inspected. 4.3. SAFETY MEETINGS Site Management and Field Safety Officer together with other TRIMEC's personnel will hold safety and security steering committee meetings to ensure a satisfactory implementation of any safety measures required and to ensure the complete understanding a commitment of TRIMEC's personnel toward safety.

    23. TRIMEC Engineering & trade ltd.co. SAFETY PLAN 5. IDENTITY BADGES 5.1. Each person shall be issued an m badge, which will include his photograph, name and personnel number. 5.2. ID badges shall be carried on the person at all times and kept safe. 5.3. ID badges shall be produced and shown to any person in authority who requests it. 5.4. Lost ID badges shall be reported to Security immediately.

    24. TRIMEC Engineering & trade ltd.co. SAFETY PLAN 6. SAFETY TRAINING 6.1. Emphasis will be placed on educating employees in safety awareness. This will be attained in a number of ways. 6.2. All employees shall receive a safety induction/orientation program of not less than 1hour. This program shall include, not exclusively, the following subjects: a) Protective clothing and equipment b) Work Permits c) Security d) Scaffolding e) Housekeeping f) Fire Prevention g) Toxic substances h) Excavation i) Electricity j) Welding k) Rigging and Lifting l) Material handling m) Confined space entry n) Accident/injury reporting

    25. TRIMEC Engineering & trade ltd.co. SAFETY PLAN The program shall be carried out by a member of the safety staff or such other person as may be authorized by Project Management. 6.3. Safety shall be an item on all project management meetings. Apart from any action the safety department may take, it shall be the responsibility of each manager to disseminate appropriate safety information at his own divisional meetings to the superintendents. 6.4. All craft general foremen and foremen shall be made familiar with their safety responsibilities, particularly with respect to the use of work permits. 6.5. Every foreman shall hold a 5 minute tool box meeting on a daily basis prior to the start of work with the whole crew. This shall take precedence over all other duties. The Foreman shall emphasize the relevant safety procedures applicable to his crew. In particular, the need for continuous awareness must be stressed. Safety staff will be available to assist with advice, but it is stressed that it is the responsibility of the foreman to carry out the talk.

    26. TRIMEC Engineering & trade ltd.co. SAFETY PLAN 6.6. All personnel shall be made familiar with the emergency evacuation procedure. 6.7. When making work assignments, the foreman shall inform his crew of the safe work practices, work methods and personal protective equipment required. He shall pay particular attention to new or inexperienced workmen. 6.8. All unsafe actions noted by supervision and/or safety staff shall be corrected at once, and the circumstances reviewed with the offending person(s). 6.9. All injuries, however minor, shall be reported by the victim to his immediate supervisor and entered in the first-aid book. The supervisor shall review the circumstances and, if possible, initiate preventative action. Any injury which requires treatment at the medical facility shall at once be reported to the safety superintendent. 6.10. Training in the use of fire extinguisher shall be provided by the safety superintendent on request or as deemed necessary.

    27. TRIMEC Engineering & trade ltd.co. SAFETY PLAN 7. WORKING DRESS & PROTECTIVE CLOTHING 7.1. Personal protective clothing and equipment is available for all MTG employees as necessary. 7.2. Minimum working dress for all employees working in the field is long pants, T-shirt and safety footwear. 7.3. All personnel working in the field and in lifting or rigging operations, shall wear hard hats. 7.4. Eye protection, hearing protection and work gloves are available as required. They may be mandatory in certain conditions. 7.5. Dust masks, cartridge respirators or breathing apparatus will be required in areas of fume, dust, vapor, gas or other toxic hazards. The type of equipment used will depend on the nature and concentration of the hazard. 7.6. Specialist crafts, such as welders and users of oxy-acetylene cutting equipment shall wear appropriate protection. 7.7. When working at heights when a proper platform, either permanent or temporary, is not practicable, safety belts or safety harness must be worn. 7.8. Gloves, leather or PVC as appropriate, shall be worn for handling operations. 7.9. Rubber boots shall be worn when working in concrete or oil.

    28. TRIMEC Engineering & trade ltd.co. SAFETY PLAN 7.10. Visitors, vendors and all other non-MTG personnel shall wear protective clothing appropriate to the area being visited. 7.11. There may be occasions when specialist items such as chemical suits may be needed. All such occasions will be considered individually. 8. HAND TOOLS 8.1. Hand tools shall be maintained in good condition. Particular attention shall be paid a) Wooden handles shall be free from splits and cracks. b) Wooden handles for hammers shall be secured with tapered wedges. c) Steel wedges and chisels shall be free from burrs. d) Wood chisels and other cutting tools shall be kept sharp. 8.2. All files and similar tools shall have proper handles, and not be used with bare tangs. 8.3. When using chipping tools the appropriate eye protection shall be used. 8.4. When using a sledgehammer and chisel, the chisel shall be held with pincers or a stiff wire loop, not by hand.

    29. TRIMEC Engineering & trade ltd.co. SAFETY PLAN 9. PNEUMATIC TOOLS 9.1. Pneumatic tools shall be in good condition. 9.2. Hoses must be kept clear of stairs and walkways, and protected from mechanical damage. 9.3. Safety clips shall be fitted to prevent the accidental ejection of the tool from the holder. 9.4. All hose connections shall be positively locked. 9.5. When using jack-hammers, chipping hammers, impact wrenches and similar equipment, eye protection and hearing protection must be worn. Consideration must also be given to other personnel working nearby. 9.6. When using jack-hammers in the horizontal, or near-horizontal mode the jack-hammer must be supported round the barrel by a rope lanyard or other means.

COLD WEATHER PROTECTION Cold weather damage most often occurs in exposed, out-of-the-way areas of the facility during weekends or other shutdown periods. Failure to identify areas likely to be susceptible to cold weather damage and improper maintenance (e.g., doors, windows, or ventilators being left open, as well as broken windows, cracks, loose siding, or similar defects in building maintenance) also contribute to cold weather damage. Therefore, it is essential to: 1. Establish a schedule for preparing the facility prior to the cold weather season. 2. Establish a weather watch with written procedures for alerting management and maintenance personnel. Procedures should be as "automatic" as possible, and should authorize certain actions at specific temperatures (actual or forecasted). 3. Arrange for watch service and maintenance personnel to be available during expected cold snaps. 4. Develop procedures for repairing or replacing damaged equipment and safely

COLD WEATHER PROGRAMS FOR FIRE PROTECTION SYSTEMS Burst pipes, frozen water lines, and cracked sprinkler heads in fire protection systems are the most frequently reported problems during cold weather. One DOE facility recently reported 20 failures of their fire protection system because of cold weather problems. Expensive repairs to the system, facility buildings, and equipment may be required as a result of such failures. More important, however, are the facility's vulnerability to fire and the increase in potential worker hazards. Recommendations 1. Maintain extra heat during periods of extreme cold, especially during idle periods, to keep the sprinkler piping from freezing. Check the heating system to ensure that it is delivering sufficient heat to all areas, particularly at night. 2. Search for isolated drafts or air leaks, particularly in infrequently visited areas and in spaces where sprinkler pipes are installed. Look specifically for places where cold air could enter and eliminate even small openings. Keep all doors, especially large shipping doors, tightly closed. 3. Increase surveillance of building pipelines, flowlines, and safety-related equipment during periods of extreme cold. Provide sufficient watch service to ensure that all plant areas can be visited each hour.

    32. TRIMEC Engineering & trade ltd.co. SAFETY PLAN 4. Check room temperatures, and use temporary heaters in those areas where temperatures drop below 40 degrees F. 5. After a prolonged period of abnormally cold weather, make drain tests of sprinkler risers wherever possible to determine if underground mains are frozen. Open the drain wide, let it run for 30 seconds or more, then shut it off. If the pressure fails to return to normal promptly, clear the mains of ice as soon as possible. 10.2. COLD-WEATHER PROGRAMS RELATED TO HAZARDOUS MATERIALS When cold weather causes pipes and hoses to burst, valves to break, or containers to crack, potentially hazardous material may be released. During the winter months, cycles of freezing and thawing may also weaken valves and cause them to rupture.

    33. TRIMEC Engineering & trade ltd.co. SAFETY PLAN Recommendations 1. Ensure that all containers used for hazardous or toxic materials are properly stored, and inspect them for deterioration prior to handling. If containers become brittle (due to the combination of chemical attack, freezing temperatures, and ultraviolet light), they may break when moved. 2. Liquids should not be permitted to remain in unheated process lines during periods when production has been stopped. All lines should be drained and purged to prevent future line breakage due to freezing temperatures. 3. Ensure that piping and valves (particularly check valves and dump valves) in systems that carry hazardous or toxic substances are properly insulated. Install insulators such as heat blankets, heat tape, or frost boxes, as appropriate. 4. Inspect all anti-freeze loop valves to ensure that they are in the open position. Chain and lock them in the open position if possible.

    34. TRIMEC Engineering & trade ltd.co. SAFETY PLAN COLD WEATHER PROGRAMS FOR BUILDINGS AND EQUIPMENT Lack of heat frequently causes building and equipment problems in severely cold weather. However, inadequate communication among personnel, inadequate procedures, and inadequate design and maintenance directly contribute to cold weather damage. When a building at one facility was damaged due to a broken sprinkler system, investigators found that the maintenance staff had not turned on the HVAC (heating, ventilation, and air conditioning) system and the operating staff had allowed outside air ducts to remain open. Although the cold air that entered the building caused sprinkler heads to break, it was the communication failure between maintenance and operating staff and their lack of awareness concerning the impact of severely cold air that led to the damage. In another incident, approximately 19 hours of reactor operation were lost because a timer failure allowed condensation to freeze in the instrument air lines, which caused a loss of air to the reactor building ventilation exhaust valve. A loose solder connection was a contributing cause of this incident.

    35. TRIMEC Engineering & trade ltd.co. SAFETY PLAN Recommendations 1. Ensure that operations/maintenance personnel communicate relevant cold weather information regarding the status of equipment, heating, ventilation, etc. Communicating such information is imperative during shift turnovers when extremely cold weather is anticipated. 2. Include specific checks for adequate cold weather protection in written equipment maintenance procedures. Follow any special manufacturer- recommended procedures related to seasonal changes (e.g., using oil of different viscosity). 3. Evaluate equipment design problems that are exacerbated by cold weather. Replace, repair, or re-design equipment and systems as appropriate.

    36. TRIMEC Engineering & trade ltd.co. SAFETY PLAN 11. SCAFFOLDING 11.1. Every scaffold must be checked by a foreman or supervisor before use. 11.2. All scaffold must be erected and maintained in accordance with established standards. 11.3. All scaffold shall be of the tubular steel or unit type. 11.4. Handrails and toe-boards shall be fitted to all scaffolds more than 2 meters high on All open sides. 11.5. Scaffold planks shall be not less than 40 mm thick and be supported at distances of not more than 2 meters. Planks shall not project more than 30 cm beyond the end support. 11.6. The minimum width of a scaffold platform for access only shall be 45 cm. As a working platform, or if materials are to be placed on it, the minimum width shall be 90 cm.

    37. TRIMEC Engineering & trade ltd.co. SAFETY PLAN 11.7. Tubular scaffolds shall have access ladders, properly secured, to ,provide access. Unit scaffolds may be fitted with integral ladders, otherwise they also must be provided with them. Access by climbing the structure is prohibited. 11.8. All stands and uprights must be supported by base plates, and also soleplates if necessary. 11.9. Scaffolds must be secured to the structure at intervals not exceeding 10meters horizontally and 8 meters vertically. In certain cases struts may be used instead. 11.10. All scaffolding materials must be in good condition. Cracked and split planks and damaged or bent tubes, shall be removed from site. 11.11. Rolling scaffolds shall have a height not greater than three times the shortest base length. 11.12. Rolling scaffolds shall only be used on firm level surfaces, e.g. concrete, tarmac, steel. They shall not be used on sand or soft material. The wheels shall be chocked or locked when the scaffold is in use. 11.13. No person shall remain on the scaffold whilst it is being moved.

    38. TRIMEC Engineering & trade ltd.co. SAFETY PLAN 12. LADDERS 12.1. Ladder Safety: When and Where Attention to safety is part of using ladders correctly--anywhere and anytime. Using ladders safety involves proper procedures when preparing to work, going up a ladder, coming down a ladder, working while on a ladder, moving a ladder, and assisting someone else on a ladder. 12.2. Taking a Good Look - The Critical Safety Measure Visual inspection of equipment and the work area prior to beginning the task is critical to using ladders safely. Ladders should be inspected for defects and to assure that they are tall enough for the job at hand. You should be able to reach what you are doing comfortably; ladders for roof work should project 3 feet above the eaves, gutter, or roof line. Before working outside, check for live power lines. If present, have the power company de-energize them or cover the lines with insulation, if possible. If you must work in the vicinity of live lines, do not use a metal ladder (or wooden ladder with metal side rail reinforcement wires). Use a nonconductive ladder, and be sure to check overhead before moving the ladder to ensure a safe working distance from power lines.

    39. TRIMEC Engineering & trade ltd.co. SAFETY PLAN Before using ladders inside or outside, make sure you are aware of any dangers from overhead obstructions which could result in head injury. Finally, notice where the feet of your ladder will be standing. Look for a secure, even surface. If you will be stepping off the ladder onto an uneven, wet, or otherwise problematic surface, be careful to prevent injury. 12.3. Up and Down Ladder climbing should be a deliberate action where full attention is applied. Considering it second nature is inviting a safety incident. Bad climbing techniques, such as not squaring the body between the side rails or not gripping with both hands, are common safety violations. Missteps occur often and produce injuries such as sprained wrists or ankles, and even broken backs. Shoes with heels help prevent feet from slipping through the rungs. Inattentive workers strike their heads on overhead ducts, beams, pipes or valves, or are unable to catch themselves after a slip because both hands are full of materials for the job. Workers climbing ladders must be aware of the space above and have their hands free to climb and handle problems as they arise. Tools should be carried in a pouch or holster, and heavy objects can be raised with a hand line.

    40. TRIMEC Engineering & trade ltd.co. SAFETY PLAN 12.4. Working Safely Above Ladder-related injuries also occur after the climbing takes place. Accident records indicate that workers fall off ladders while stretching to reach their work. Several workers have fallen off ladders when reaching to drill overhead. The tendency to overreach beyond a safe distance (arm's length without stretching) is generally caused by one or both of these failings: picking a ladder that is too short or failing to climb down and move the ladder as necessary. To avoid these highly dangerous situations, choose a ladder of proper length. Properly position the ladder under your work, do not shift from side to side, and climb down and move it as required. Consider scaffolding if the job requires excessive ladder moving. 12.5. The Unsecured Ladder Workers are injured when unsecured ladders slip, shift or fall. First, an incorrect angle of the ladder against the wall can cause a problem. The distance of the base of the ladder from the wall should equal one quarter the length of the ladder (72.5 degrees). Tying off near or at the top adds extra stability, and placing the feet carefully on a dry, firm surface (avoid ice, snow, and water) will help assure a steady structure from which to work.

    41. TRIMEC Engineering & trade ltd.co. SAFETY PLAN 12.6. Stepladders - More Dangerous Than They Appear Stepladders are deceptive. They look stable because of their A-frame construction, but in reality have caused numerous injuries to workers using them. Several stepladder injuries occurred when people in a hurry used unopened stepladders to reach something. Even when opened, stepladders are not stable enough for workers to stand on the top step. Stepladders cannot be used like regular ladders, and must be fully opened with spreaders locked and all four feet firmly planted before they are safe for use. Never stand on the top step/platform of a stepladder. The highest safe step is clearly marked. 12.7. Ladder Parts Not Intended To Be Stood On • A ladder top cap; the top step of a stepladder or trestle ladder; the pail shelf of a self supporting ladder. • The rear braces of a self-supporting ladder, unless the braces are designed and recommended for that purpose by the manufacturer. • The top step of the extension section of an extension trestle ladder. • The top cap or step of a combination ladder when it is used as a self-supporting ladder. • The top three feet of an extension ladder, or anywhere above the support point.

    42. TRIMEC Engineering & trade ltd.co. SAFETY PLAN 13. JUMPING & FALLING Many injuries are caused when workers jump or fall off cranes while descending from the cab. In most of the cases studied, the workers were inexperienced and did not realize the dangers involved in jumping from even a short height. Workers sustained sprained ankles and twisted and sprained knees after jumping instead of using the ladder. Some workers were injured when they fell while climbing up to the cab and carrying items in their hands. Those workers were often unable to catch themselves to avoid a fall when their feet slipped. Recommendations 1. Use the ladder when climbing up to the cab. Keep a firm grip on the handrails to avoid falling if your feet slip. 2. Use the ladder to climb down - do not jump. 3. Avoid handling or carrying objects while climbing up or down the ladder. Anything that cannot be transported via pockets or belts should be hauled up using a hand line. 4. Take precautions to ensure that loose clothing, such as coats or rain gear, does not catch on bolts or handles and cause a fall.

    43. TRIMEC Engineering & trade ltd.co. SAFETY PLAN 14. WORK PERMITS 14.1. Work Permits are required for all work carried out on all operational equipment - vessels, pipes, pumps, tanks etc. - which have been in operational use. 14.2. Work Permits are normally issued by the Process Department of the Company. 14.3. In certain cases it may be that the responsibility for the issue of such permits rests with CASCON. The procedures given below shall be followed. 14.4. Types of permit. 14.4.1. Hot Work Permit This is required for any work which entails the production or use of a source of ignition - welding, cutting, grinding, heat treatment, internal combustion engines, electrical equipment (unless intrinsically safe) etc. 14.4.2. Additional supporting permits may be required, depending on the nature of the work. In all cases these supporting permits are only a clearance for the issue of the main Hot Work Permit. The additional permits are: • Electrical Lock-out • Vessel Entry In any cases where these permits are required, they shall be attached to the relevant Hot Work Permit.

    44. TRIMEC Engineering & trade ltd.co. SAFETY PLAN 14.5. Issuing of Permits - Hot Work 14.5.1. Part 1 of the permit is completed by the Supervisor or Superintendent of the section wishing to carry out the work: 14.5.2. Hot Work Permits should normally be submitted to the appropriate Company Superintendent at least 5 hours before they are required. This is to allow adequate time for preparation. 14.5.3. The Company Superintendent shall review the "conditions" in Part 2, in conjunction with a member of the Company Safe Department and tick the "yes" or "no" box. Where necessary he shall make the appropriate arrangement for isolation, purging, lock-out etc. 14.5.4. Once the Company Superintendent and Safety Supervisor are satisfied that the equipment is safe to work on they shall sign part 2. 14.5.5. Prior to signing the permit any special conditions shall be entered in Part 3 and the results of the gas test in part 4. 14.5.6. The Crew Foreman or Leading Hand then signs Part 5 and take the top copy of the permit with him to the place of work. The second copy remains with the Company Superintendent.

    45. TRIMEC Engineering & trade ltd.co. SAFETY PLAN 14.6. Supporting Permits The supporting permits shall always be issued prior to the Hot Work Permits, and shall be attached to them. 14.7. Cancellation of Permit The permit shall be canceled under any of the following conditions: i. If the issuing Superintendent / Supervisor so instructs. ii. If safety so instructs. iii. If the area conditions change, iv. If the nature or scope of the work changes. 14.8. Alteration of Permits Under no circumstances shall the Permit Receiver alter the conditions set out in the permit. Should he wish to change the scope or type of the work, he shall refer to the issuing Authority. 14.9. Gas Testing Gas testing will normally be carried out by Company Safety Staff. In certain cases other persons shall be instructed in the methods of gas testing and be authorized to carry them out.

    46. TRIMEC Engineering & trade ltd.co. SAFETY PLAN 15. WELDING & GAS CUTTING In any petro - chemical environmental welding and gas cutting are a major firepotential. The following rules must be rigidly adhered to 15.1. All persons engaged in welding and cutting must wear the appropriate protective clothing, welder’s hood and screen or cutting goggles, welders jacket, gloves and safety footwear. 15.2. Before cutting or welding any existing pipe, vessel or other equipment a gas test must be carried out and a work permit issued. 15.3. Unless the equipment can positively be identified as being gas free it shall only be cold cut. 15.4. The area shall be free of all combustible material. 15.5. All cutting and welding equipment shall be in good condition. Welding cables with damaged insulation shall be property repaired. 15.6. Ground return clamps shall be in good condition and applied as close to the welding point as possible. 15.7. Gas and oxygen hoses for gas-cutting equipment shall be color coded. They shall be free from cracks or other damage and fitted with blow-back preventers. 15.8. Gas cylinders shall always be secured in an upright position, either in trolleys or frames, or to fixed steel uprights.

    47. TRIMEC Engineering & trade ltd.co. SAFETY PLAN 15.9. When being transported, all valves and connections shall be removed from cylinders and the safety caps fitted. 15.10. When not in use, the cylinder valves on gas cylinders shall be closed. 15.11. Welding generator sets shall be grounded when in use. 15.12. Valve keys for gas cylinders shall be kept with the cylinders when in use. Recommendations 1. Provide additional training regarding interpretation and implementation of procedures and supporting requirements to personnel who issue work permits, including safety, fire, and health inspectors. 2. Require safety, fire, and health inspectors to verify work permit conditions before work begins. This process should include verification of any work control modifications stemming from changes in conditions under which the permit was originally issued. 3. Provide hazard communication training, as well as training in fire protection requirements and fire prevention practices, to facility and subcontractor personnel involved in cutting and welding activities.

    48. TRIMEC Engineering & trade ltd.co. SAFETY PLAN 4. Ensure that fire watch procedures are adequate and that fire watchers are properly trained in their duties. 5. Take special precautions for confined space hazards associated with cutting / welding activities, and ensure that fire watch and emergency response team entry into confined spaces is in accordance with OSHA requirements and those of the American National Standards Institute (ANSI).

    49. TRIMEC Engineering & trade ltd.co. SAFETY PLAN ELECTRICAL INSTALLATION 16.1. Except electricians nobody is allowed to modify or repair electrical installations. 16.2. If you see any defects in the electrical installations that may prove harmful to human lives or the equipment, inform immediately your Supervisor. 16.3. Access to transformer stations and electric generators must be kept clear at any time. Do not obstruct it in any way, 16.4. Unauthorized persons are not allowed to enter electrical operational installations or mobile control stands. 16.5. Defected and damaged electrical equipment and tools must not be used before being repaired. 16.6. If you defect any electrical installations these must immediately put out of service. Inform your Supervisor immediately.

SAFETY PLAN General categories of causes found in the shock and burn reports include: 1) Contact with faulty (shorted) equipment (e.g., plugs, power tools, and welders). 2) Improperly wired equipment. 3) Failure to implement lockout/tagout procedures and/or use adequate protective equipment. 4) Inattentiveness and carelessness, including: a) use of uninsulated tools b) use of wrong instrument/meters/tools c) failure to pre-verify volt/amp meter operation d) allowing tools to slip onto live circuits e) wearing jewelry (e.g., rings, watches, bracelets) while working on live circuits.

SAFETY PLAN 5) Manufacturers' defects and aging equipment. 6) Inadequate procedures governing excavating and concrete cutting/drilling. 7) Failure to recognize the dangers of large capacitors. Shock and Burn Recommendations 1. Review programs for the inspection and/or repair of portable electrical equipment for completeness and effectiveness. 2. Review policies concerning work permits on "live" circuits with a goal of reducing the frequency of such work. 3. Emphasize electrical worker training in certain areas such as the following: • Lockout/tagout practices • Use of protective equipment • Use of insulated tools • Minimum approach distances • Meter selection/testing/use

SAFETY PLAN • Electrical rescue/CPR • Potential dangers involving metal tapes/"fish" tapes • Include a pre-task review of the following for supervision of selected electrical work. • Goals of the task • Task methodology (live vs. lockout/tagout) • Qualifications of assigned personnel • Proper instrumentation/tools • Adequate protective equipment and usage • Methods of preventing a fall should a shock occur 4. Perform an inventory of energized electrical circuits with a goal of disconnecting unused circuits from the source and removing the wiring.

    53. TRIMEC Engineering & trade ltd.co. SAFETY PLAN General categories of causes found in the fire and explosions reports include: 1. Failure to include appropriate equipment in Preventative Maintenance (PM) programs. 2. Failure to have an electrical safety program and/or implement established principles. 3. Lack of knowledge by key individuals of the location of critical shut-off breakers and switches. 4. Unattended energized equipment and appliances. 5. Equipment age. 6. Improperly sized overload devices/no lightning arrestors or voltage spike suppressor circuits. 7. Inadequately trained and qualified high voltage electrical workers. 8. Failure to develop work packages or require safe work/hazardous work permits for tasks involving energized high voltage circuits. 9. Failure to fully implement lockout / tagout procedures.

    54. TRIMEC Engineering & trade ltd.co. SAFETY PLAN Fire and Explosion Recommendations 1. Review electrical preventative maintenance programs to ensure all necessary equipment is included (e.g. connections). 2. Identify old equipment and develop a program/schedule to replace the identified equipment (e.g. transformers, large capacitors, ballasts). 3. Know locations of critical shut-off and kill switches, and ensure that they are clearly identified. 4. Check trailers and modular buildings for electrical panel quality and size, appliance loads, loose connectors, and faulty electrical heaters. 5. Develop a program to ensure that all nonessential unattended appliances are turned off. 6. Control all high voltage tasks with a work package that includes a hazardous/safe work permit. 7. Protect deluge and sprinkler systems over switchgear, transformers, or other high voltage electrical equipment from accidental activation (e.g., manual control). 8. Review the adequacy of the current number of lightning arrestors and/or voltage spike suppression circuits. 9. Make fire extinguishers available at remote job sites and in all vehicles.

    55. TRIMEC Engineering & trade ltd.co. SAFETY PLAN Failure to implement lockout/tagout procedures resulted in the death of one worker and injury to another when an unlocked/untagged electrical solenoid valve was inadvertently opened, allowing natural gas to be released into the work area which subsequently ignited and exploded. Those most at risk from electrically initiated fires and explosion are the workers in the immediate area (i.e., electricians, technicians, mechanics, etc.). However, fires that spread (and larger explosions) have the potential to affect others as well, including the firefighters charged with extinguishing the fires and the workers required to cleanup or return the system to an operable condition. Other Hazards of Electrical Work Two deaths involving electrical workers did not result from either direct electrical contact or a fire or explosion. These deaths were the result of 1) falling while removing old conduit from a building being decommissioned and 2) being crushed by a tree being felled near a power line. Although neither involved electrical safety issues per se, additional "general worker training" might have reduced the likelihood of these and other similar incidents.

    56. TRIMEC Engineering & trade ltd.co. SAFETY PLAN General Recommendations Employees should be provided training that covers information regarding electrical risks such as inadequate grounding and reverse polarity and likely electric shock producing equipment, including extension cords, plugs, and portable power tools. The dangers of energized and unattended appliances should be stressed in this training, as well as the theory behind lockout and tagout procedures. Employees working with electricity should also be informed on how to recognize electric shock victims, safe methods of rescue, and cardio-pulmonary resuscitation. 17. ELECTRICAL LOCK-OUT 17.1. No work shall be carried out on electrical, or electrically driven equipment unless the power supply has been disconnected physically or it has been locked out at the switchgear. 17.2. An electrical lock-out form, shall be used to record the locking out. 17.3. Locking out shall only be performed by a qualified electrician.

    57. TRIMEC Engineering & trade ltd.co. SAFETY PLAN 18. LIFTING & RIGGING 18.1. All cranes and lifting equipment shall be in good condition. 18.2. Cranes shall be operated in accordance with standard procedures and the manufacturer's handbook. No unauthorized modifications e.g. additional counterweights - shall be permitted. 18.3. Under no circumstances shall the safe working load indicated on the load rating chart be exceeded. 18.4. The crane operator must be properly trained and experienced for the class of crane he is operating. 18.5. A rigger / banksman shall be appointed. He shall use only standard crane signals and shall be the only person the crane operator shall acknowledge, other than for “Emergency Stop". 18.6. Repairs, maintenance and refueling shall not be carried out while the crane is in use. 18.7. Great care shall be taken when operating in the vicinity of overhead power lines.

    58. TRIMEC Engineering & trade ltd.co. SAFETY PLAN 18.8. All crane hooks shall be fitted with safety catches. 18.9. "Hook riding" is prohibited. 18.10. Long or unwieldy loads shall be controlled by tag-lines. 18.11. Suspended cradles and bosun's chairs shall always be controlled by two tag-lines. 18.12. Truck-mounted cranes shall always have the outriggers extended and jacked down when operating in the stationary mode. 18.13. When truck-mounted or rough terrain cranes are traveling with a load, the load shall be kept as close to the ground as possible and be steadied from swinging by tag lines. 18.14. When a truck mounted crane is traveling the slewing lock shall always be in the "locked" position. 18.15. Under no circumstances shall the operator leave the control whilst a load is suspended on the hook. 18.16. Care shall be taken when operating in windy conditions. Wind speeds in excess of 30 km/h may justify the crane being taken out of operation.

    59. TRIMEC Engineering & trade ltd.co. SAFETY PLAN 18.17. All lifting gear-chains, wire ropes, hooks, slings etc. - shall be in good condition. the rigger shall inspect each item before use. 18.18. Slings, ropes and chains shall not be knotted, nor shall they be used as tow ropes. 18.19. All slings, hooks etc. shall be attached to the load by a competent rigger. 18.20. Multi-legged slings shall normally be used with a maximum angle between the legs of 90°. 18.21. The rigger shall be clearly identifiable by the crane operator, and shall only use standard crane signals. 18.22. When not in use, all lifting gear shall be stored in a clean dry place where it is safe from physical damage. Recommendations 1. Keep your fingers and arms from between the sling and the load and from between the sling and the hook. Gloves are recommended. 2. Examine the surrounding area carefully before beginning maintenance activities. Look for obstructions in the area where you will be bending, straightening, or walking. 3. Follow lockout/tagout procedures (turn the crane off, lock the main switch in the off position, and tag of flag it), and place an OUT OF ORDER sign on the crane in a highly visible place before starting any maintenance activity.

    60. TRIMEC Engineering & trade ltd.co. SAFETY PLAN 19. USE OF COMPRESSED AIR 19.1. Use of compressed air for cleaning or drying shall only be permitted when other more acceptable methods are impracticable. 19.2. Under normal conditions the maximum pressure permissible for such purposes shall be 0.7 bar / 10 psi. Air lines shall be fitted with an appropriate pressure regulator. 19.3. Under no conditions shall compressed air be used to blow dust or dirt off an employees skin or clothing, or for cooling purposes. 19.4. Any person handling compressed air shall wear eye protection. 19.5. Compressed air hoses used for cleaning or drying shall be fitted with a proper nozzle which includes an on/off valve. 19.6. Under no circumstances, except in an emergency, shall the air supply be cut off by kinking the hose. 19.7. Hose couplings shall be fitted with pins or other means to prevent accidental disconnection. 19.8. Horseplay with compressed air is extremely dangerous and is prohibited.

    61. TRIMEC Engineering & trade ltd.co. SAFETY PLAN 20. WORKSHOP PRACTICE 20.1. Workshops shall be kept tidy and free from waste materials, rubbish etc 20.2. Gangways shall be clearly marked and kept free of obstructions. 20.3. Each machine shall have its own clearly marked area. 20.4. Where necessary guards or screens shall be placed to prevent particles and chippings from machines striking persons on adjacent machines or in the gangways. 20.5. All rubbish shall be placed in bins or drums, which shall be emptied regularly. Oil rags shall not be allowed to accumulate because of the risk of auto-ignition. 20.6. High pressure compressed air shall not be used to blow swarf, etc. 20.7. Where overhead gantry cranes are installed, they shall only be operated by competent persons. They shall be fitted with travel alarms. 20.8. Oil spillage shall be cleaned up immediately. 20.9. Fire equipment must not be obstructed.

    62. TRIMEC Engineering & trade ltd.co. SAFETY PLAN 21. DRIVING RULES AND REGULATIONS 21.1. Drivers should make a daily inspection of their vehicles before starting a shift. 21.2. It is the responsibility of the driver to inform his senior about any fault in the vehicle appeared during the course of work. These faults must be repaired immediately. 21.3. No person should be permitted to remain in or on a truck being loaded by crane. Do not overload material beyond the capacity of truck. Material loaded should be within safe weight limit for the truck and should not project beyond the truck body in such a manner as to present a hazard to other vehicles. Load projecting over the end of the truck should be warned with a red flag and lighted at night. Loose material should be covered to prevent its falling down. Soil, concrete or stones must be loaded in the center of the body to keep the vehicle balance favorable. Select always smooth places for loading: do not unload material by parking your vehicle on a sloping surface. 21.4. Road signs must be observed at all times. Ignorance of traffic signs is most dangerous and constitutes a breach of traffic rules. Action may be taken against defaulting drivers under the Safety Rules and Regulations.

    63. TRIMEC Engineering & trade ltd.co. SAFETY PLAN 21.5. Drive within speed limit of the area concerned. Always drive slowly on road crossings, in curves, on link road and rough passageways. On roads being newly constructed or repaired, careful and slow driving is required. 21.6. Always adopt recommended roads which are traffic sign posted. It is dangerous to use short-cuts or deserted passageways. 21.7. For safe backing keep mirrors in proper direction. Do not depend upon mirrors at night time, because drains or ditches cannot be seen easily in mirrors, Vehicle must be backed under the direction of a signal man if the driver cannot clearly see the area to the rear of his vehicle. 21.8. Never light a cigarette when fueling. Do not enter the petrol pump boundary with a lit cigarette. Always observe NO SMOKING 21.9. All vehicles must carry useable fire extinguishers. 21.10. Always park your vehicle within the recommended parking areas. No vehicle shall be left unattended until after the motor has been turned off, the key removed and parking brake securely set. If the parking area is sloped, place stones or bricks against the wheels and turn the front wheels accordingly.

    64. TRIMEC Engineering & trade ltd.co. SAFETY PLAN 21.11. Lights play a vital part in the prevention of accidents. Never use a vehicle with defected lights. Periodically, drivers must check the battery, indicators, head lights and rear lights. Head lights beams must be depressed when approaching other vehicles. 21.12. Drivers must under no circumstances hand the key of their vehicles to an unauthorized man without the permission of their senior. In case of any accident, they will otherwise be responsible. 21.13. Trucks must be serviced after a specific limit of mileage or time. Drivers are to inform their seniors when the service of their vehicles is due. In case of any repair needed, the drivers should bring this necessity to the notice of their senior. 21.14. Safety Officers may test the drivers. In case drivers ignore the basic principles of driving or do not know the road signs, the officer can request their senior to change their job or dismiss them. 21.15. Drivers are strictly warned that they must properly look behind or under the vehicle before starting or backing.

    65. TRIMEC Engineering & trade ltd.co. SAFETY PLAN 21.16. The following precautionary measures should strictly be complied with: a) Road signs should be observed at all times. b) No vehicle should be driven exceeding the speed limit in site area. c) High tension wires running through the premises are provided with warning signs. They must be strictly observed during transporting bigger equipment through that area for height clearance. d) Stop at "blind corners", blow horn before proceeding and approaching to the people walking on foot. e) Vehicles must be parked only in authorized parking places. f) Employees and others are strictly warned to walk on footpaths wherever provided. g) Truck drivers are strictly warned not to carry people behind on the body. h) In storm or rainfall, the speed must be decreased. In such circumstances use lights and keep always towards the right hand side. i) Be careful, while over-taking. Not try to proceed before getting signal from vehicle driver going ahead.

    66. TRIMEC Engineering & trade ltd.co. SAFETY PLAN 22. CRANES & OVERHEAD POWER LINES 22.1. Whenever overhead power lines are in the area or vicinity of construction operations which involve the use of cranes or high vehicles such as backhoes or tipping trucks, the lines shall be protected by "goal posts". These shall be firmly installed and clearly visible. The cross member may be wire rope or a rigid beam, as practicable. 22.2. Goal posts normally shall be set a minimum of 30 meters on each side and each pair shall be far enough apart to span the full width of the roadway. 22.3. The height of the crosswire or crossbar shall vary accordingly to the voltage of the power lines, but in any case shall not be less than 1.5 meters lower than the power line. Other distance shall be as follows: 50 kV to 345 kV - 3 meters 345 kV to 750 kV - 5 meters 22.4. When a crane is transiting the power lines a banksman / rigger shall be in attendance to check the clearance of the boom.

    67. TRIMEC Engineering & trade ltd.co. SAFETY PLAN 22.5. No crane or similar equipment shall be operated within 3 meters of any electrical , transmission lines. 22.6. Cranes shall not be parked under electric power lines since there is the possibility of a considerable electrostatic charge building up. 22.7. All power lines shall be considered energized unless certified otherwise by a responsible representative of the power company. If the work is to be carried out within the danger distance of a de-energized line, the line must have a grounding cable of adequate capacity immediately to trip out the system should it accidentally re-energized. Recommendations Lower the boom when in transit. 2. Look carefully before moving the boom, particularly in congested areas of poor visibility, and request assistance if you cannot see clearly. 3. Consider any overhead wire to be energized unless the line owner or utility company tells you that it is not energized. 4. Maintain a minimum clearance of 10 feet between the power line and any part of the crane or load when working around energized power lines rated at 50 kV or below.

    68. TRIMEC Engineering & trade ltd.co. SAFETY PLAN 5. Maintain a minimum clearance between the lines and any part of the crane or load of 10 feet plus 0.4 inch for every 1 kV over 50 kV, or twice the length of the line insulator, for voltages above 50 kV. 6. When in transit, with no load and the boom lowered, maintain at least 4 feet of clearance from lines rated less than 50 kV, 10 feet from lines rated over to kV, up to and including 345 kV, and 16 feet from lines rated up to and including 750 kV. 7. Notify your manager whenever the crane will be in use, so that you will have adequate supervision.

SAFETY PLAN Additional Good Work Practices The majority of crane-related accidents can be avoided if workers and supervisors take the following safety precautions before beginning work. Supervisors 1. Ensure that workers are trained in all crane safety procedures. 2. Be certain that all hoisting equipment is in good shape and rated for the weight to be lifted. 3. Ensure that ladder steps, platforms, and walkways have nonskid surfaces. Keep walking surfaces clean and free of mud and ice. Workers 4. Do not exceed the rated load capacity and understand the load charts; exceeding load capacity may cause the crane to tip over. 5. Use a signal person if you cannot see the load you are trying to move or maneuver. 6. Never suspend a load over or close to people. Make sure the area of travel or boom movement is clear before maneuvering a load. 7. Keep clear of the crane swing radius when lifting is being done. Do not stand under suspended loads or in the load's line of motion.

    70. TRIMEC Engineering & trade ltd.co. SAFETY PLAN 23. HEAVY EQUIPMENT 23.1. This is defined as trucks, dozers, mechanical shovels, side booms, cranes, graders and similar equipment. 23.2. All the equipment must be properly maintained and in good condition. 23.3. All operators shall be properly trained on the type of equipment they are operating, and shall hold the appropriate license, if required by law. 23.4. Wheel mounted equipment, other than trucks, shall be fitted with a reversing alarm. This applies particularly to mechanical shovels, truck cranes and fork lift trucks. 23.5. Passengers shall only ride on equipment if there is a properly installed seat for them. 23.6. Load-carrying vehicles shall not be loaded beyond their rated capacity 23.7. Operators shall not refuel equipment with the engine running. 23.8. No equipment shall enter a restricted area without a work permit.

Inspect the crane before use, checking for deformed or cracked hooks and wear on the host chain, sling, and associated equipment. Ensure that the crane and associated hoisting machinery is inspected annually by a competent person or by an agency recognized by the U.S. Department of Labor. 2. Ensure that the load is well secured and balanced in the sling or rigging before it is lifted more than a few inches from the ground. 3. Position the hood directly over the center of the load to minimize swinging. 4. Keep the hook's load centered, and avoid point loading the hook unless it is designed for that. 5. Do not attempt to manually shift, turn, or redistribute a load once it has been lifted, since this increases the risk of accidents and injuries. If the load is off center or looks as if it will slip, ask the operator to lower the load back to the ground before you approach to make adjustments. 6. Wear safety shoes and hard hats, and keep clear of the suspended load. 7. Check the area in the path of the moving load to ensure that it will not hit anything, especially if it swings suddenly. 8. Move the loaded hook smoothly, avoid sudden accelerations or decelerations, which can cause the load to swing.

    72. TRIMEC Engineering & trade ltd.co. SAFETY PLAN 24. MACHINERY GUARDING 24.1. All machinery shall have the dangerous parts guarded. Particular attention shall be paid to gears, chain and sprocket drives, belt drives, shafts, spindles, chucks and similar rotating parts. 24.2. All powered machinery shall have the power source shut off or disconnected prior to maintenance or repair work on the moving parts. 24.3. Grinding machines, pillar drills, lathes and similar machines shall have guards round the workface to contain swarf and chippings. 24.4. Woodworking planers and spindle moulding machines shall have the cutters guarded. 24.5. All machinery shall have an emergency stop button of the "mushroom" type conveniently located where the operator can easily reach it from his normal working position. It shall be colored red. 24.6. All swarf, chippings, sawdust and other debris shall be regularly removed and not allowed to build up. 24.7. Where cutting oils are used, suitable trays shall be provided to avoid spillage. 24.8. Suitable eye protection shall be provided and used at all cutting, drilling, planning and other machines producing particles of debris.

    73. TRIMEC Engineering & trade ltd.co. SAFETY PLAN 24.9. All operators of machines shall be properly trained and instructed in their use. 24.10. Machinery operators shall wear clothing with either short sleeves or closely buttoned cuffs. No ties, scarves and loose head cloths shall be worn. Except in exceptional cases, gloves shall not be worn. Recommendations Managers and supervisors should follow these procedures and practices in areas where forklifts are used: 1. Ensure that the forklift's rated capacity is adequate for the type of work being performed and that all forklift operators are trained and certified. Training should include the general equipment category, as well as instruction in how equipment will be used at the facility and how specific pieces of equipment should be operated. 2. Study vehicle and pedestrian traffic. Explore ways to separate the two in time and distance, and review compliance with existing traffic rules. Determine whether employee training programs on vehicle and pedestrian safety are adequate. Implement changes as necessary. 3. Make operators aware of the severely limited visibility in some models of forklifts, and instruct them to use extreme caution when driving in reverse. Consider the use of ground guides, rear-view mirrors, spotters, or other aids to increase visibility.

Determine whether an area should have physical barriers such as ramps, raised concrete staging areas, and / or heavy-gauge safety chains in front of dock openings. Add a "warning track" of yellow paint on the floor near dock openings. 5. Ensure electrical equipment is protected from physical damage by location or substantial guards. Operators of forklifts and other heavy industrial equipment should follow these procedures and practices in areas where forklifts are used: 1. Consider the noise level in the surrounding environment. Do not assume pedestrians or bystanders are able to hear the back-up alarm. In addition, never assume pedestrians or bystanders are aware of the presence of heavy equipment and/or the intended direction of travel. 2. Use ground guides when visibility is limited. 3. Assure the path is clear prior to traveling. Note and take precautions near hazardous areas.

    75. TRIMEC Engineering & trade ltd.co. SAFETY PLAN 25. MATERIAL HANDLING & STORAGE Fire and explosion are only two of the many hazards that may result from improper storage of chemicals. A review and analysis of the storage-related incidents identified the following principal causes: 1. Improper or non-existent labeling of chemicals in storage. 2. Storage of chemicals beyond the recommended shelf-life. 3. Degradation of chemical storage containers. 25.1. All materials shall be properly stored on racks or pallets. 25.2. Gangways shall be left to allow easy access to all materials, either by personnel or mechanical lifting equipment as appropriate. 25.3. Adequate fire-fighting equipment must be provided. Such equipment must be readily accessible and not obstructed by the materials. 25.4. Manual handing and lifting must be done correctly, using a straight back and bending the legs. 25.5. Only trained and competent operators shall use fork lift trucks.

    76. TRIMEC Engineering & trade ltd.co. SAFETY PLAN 25.6. Fork Lift Truck shall not be loaded so as the operator cannot see ahead. If this is not possible the vehicle shall travel in reverse. 25.7. Fork Lift Truck shall only travel with the load as near to the ground as possible, never with it elevated. 25.8. When loaded Fork Lift Trucks are traveling up or down ramps they shall always do so with the load up-ramp. 25.9. Fork Lift Truck shall always be fitted with reversing alarms. 25.10. Passengers are prohibited on Fork Lift Truck. 25.11. Volatile and flammable materials shall be stored in a separate, well-ventilated building away from the main stores. 25.12. Compressed gas cylinders shall be stored in accordance with Section No.10. 25.13. Mechanical lifting equipment shall only be used within its rated capacity. 25.14. When opening wooden packing cases, ensure that all nails are either pulled or bent over. 25.15. Always wear work gloves when handling materials.

    77. TRIMEC Engineering & trade ltd.co. SAFETY PLAN 26. HANDLING & STORAGE OF COMPRESSED GAS CYLINDERS 26.1. Protective caps must be kept on all cylinders not in use. The only exception is the gas cylinders which have a raised collar to protect the valve and nozzle. 26.2. Cylinders must be secured in an upright position. 26.3. Fuel gas cylinders must be stored separately from oxygen cylinders, with suitable linear separation. 26.4. Cylinders must not be stored in direct sunlight, 26.5. Cylinders must not be stored or used in confined spaces. If stored in a building or room there must be extensive ventilation to the outside atmosphere at both ground and ceiling level. 26.6. All gauges must be accurate and in good condition. 26.7. If for any reason an acetylene cylinder is laid on it's side, it shall be restored to the upright position and may not be used for 24 hours. 26.8. When not in use, cylinder valves must be closed, hoses bled and torch valves closed. In any case, the torch shall not be left in a confined space.

    78. TRIMEC Engineering & trade ltd.co. SAFETY PLAN 26.9. Correct hose clips must be used for attaching hose. 26.10. No copper connections shall be used. 26.11. Cylinders shall be transported in trolleys or cradles, not rolled on the ground. 26.12. If cylinders are lifted to a higher elevation, it shall be done in proper lifting cradles not by “wrap-around” slings. 26.13. Cylinders with obvious signs of damage or heavy corrosion shall not be used, but removed to a safe place, de-pressured and disposed of. 26.14. On no account should oil or grease be used on valves. 27. HOUSEKEEPING 27.1. Waste materials and rubbish are a fire risk and an accident hazard. They shall be kept to an absolute minimum and placed in either scrap dumps or rubbish containers. 27.2. Waste containers shall be located at suitable places on site.

    79. TRIMEC Engineering & trade ltd.co. SAFETY PLAN 27.3. Do not allow oily rags to accumulate, they can ignite spontaneously. 27.4. Spilled liquids shall be wiped up immediately. 27.5. Do not obstruct walkways, roadways, gangways or stairways. 27.6. Do not bring more material from the materials yard than is needed. It gets in the way and leads to waste. 27.7. Return surplus materials to the material yard or store. 27.8. Ensure that tools are kept in tool boxes when not in use. 28. FIRE PREVENTION 28.1. The space between buildings shall not be obstructed, either by materials,equipment or vehicles. 28.2. Highly flammable substances - gas cylinders, gasoline and other fuels, paints and thinners etc. - shall all be stored in proper buildings well away from the rest and clearly marked. 28.3. Appropriate first aid fire fighting equipment - extinguishers hose-reels etc. - shall be located in all buildings and stores. 28.4. All fire fighting equipment shall be regularly serviced and inspected.

    80. TRIMEC Engineering & trade ltd.co. SAFETY PLAN 28.5. Fuel dispensation from tanks shall be in restricted areas, properly marked. All tanks shall be grounded. 28.6. Flammable liquids shall not be kept or transported in containers larger than 10 liters capacity. Only approved containers shall be used. 28.7. Where there is the possibility of spillage of flammable liquid, care shall be taken that it does not run into any drains or sewers. Drip trays shall be provided. 28.8. Mess halls, recreation centers and other public buildings shall be fitted with alarm bells and push buttons. 28.9. On locating a fire, person shall first of all call his seniors and/or safety officer. 28.10. Only if you can do so at no risk to yourself should you endeavor to attack the fire. 28.11. Switch of all unnecessary electrical equipment when you leave a building. 28.12. Do not use home-made, taped, electrical connections, plugs, sockets, etc. 28.13. Do not overload sockets. 28.14. Only use proper ash-trays, do not put cigarette ash or butts in the trash bin. 28.15. Security guards patrolling the buildings, offices etc. shall be particularly alert for fires. Those found in the early stages can usually be controlled.

    81. TRIMEC Engineering & trade ltd.co. SAFETY PLAN 29. SECURITY 29.1. Stores, materials, tools and equipment shall not be left in positions where they can be pilfered. 29.2. Tool boxes on vehicles shall be securely bolted to the body of the truck and kept padlocked except when actually in use. 29.3. Vehicles shall always be locked when unattended. 29.4. Unused materials shall be returned to the materials yard at the end of the day or shift unless the vehicle can be parked in a secure, locked compound. 29.5. Materials and equipment shall only be removed from the materials yard or stores under the authority of a gate pass signed by an authorized member of supervision.

Accident or Damage may be defined as any of the following: a) The bursting of a grinding wheel. b) Collapse or failure of a crane, derrick or other lifting appliance. c) Explosion or fire causing damage to building, equipment or materials. d) Major electrical short circuit or fire. e) Vehicle accidents. 30.2. All the above shall be reported as soon as possible to site management and the safety officer. 30.3. The scene of the accident shall be left unchanged until an investigation has been carried out by safety, except as is necessary to rescue personnel and prevent any aggravation of the incident. 30.4. Wherever possible, photographs of the scene shall be taken. 30.5. Statements should be taken from all witnesses and persons involved in the incident.

Issuance and Control The responsibility for the issuance and accountability control of the program rests with the Project Quality Assurance Manager. He will determine those organizational units performing activities affecting quality and shall provide accountable copies of the project Quality Assurance Program Plan to appropriate project personnel. 3.2.2. Authority for Quality Assurance Program Plan Revisions and Deviations Revisions to, and deviations from, the Quality Assurance Program Plan are authorized when recommended by the Quality Assurance Manager and approved in writing by the Project Manager. A revision is defined as a permanent change to the quality Assurance Program Plan which is distributed by the Quality Assurance Manager in the form of printed replacement pages. A Program Deviation is a temporary change to the QA Program Plan to permit a particular function to be completed on a individual case basis.

Method for Revision and Deviation A revision to, or a deviation from, the quality Assurance Program Plan is issued by the Quality Assurance Manager. Request for revision or deviation is initiated by any supervisory personnel using the change request procedure or non-conformance procedure. The request is forwarded by the initiator to the Quality Assurance Manager. Upon his preliminary approval of the request, the following steps are taken: • The request will be circulated for review and comment to cognizant management and technical personnel as determined by the Project Manager. If upon completion of the comment and the review phase, the revision or deviation request is not approved, the request will be returned to the initiator starting the reasons for disapproval. • If upon completion of the comment and the review phase the request is approved, the approved revision and deviation will be issued to all holders of the QA Program Plan. When issue, revisions will be assigned a number and effective date. When a revision is issued a new list of effective pages will be forwarded with the revisions. When issued, deviations will be assigned a number and given both an effective and expiration date. Periodically, the quality assurance manager will issue a summary list of effective deviations.

Project QA & QC Engineer Project QA & QC Engineer is responsible for the continuing development, implementation, management, surveillance and audit of the Project Quality Assurance Program. This responsibility extends into all project activities including engineering, design, procurement, manufacturing, construction, testing, assembly, erecting and commissioning. Project QA & QC Engineer will assure that the facilities are designed and built in accordance with the client's specifications and the current, safe, and industry accepted codes and practices. Quality plans are a part of the quality system in order to control the reference points of the project. It is closely related to both process and the product. The management representative is responsible for the integrity, harmony and validity of the documents related to the topics affecting quality. The procedures are prepared and implemented by the departments doing the job. The related departments are responsible for the implementation. Management representative is only responsible for the continuity. The documents related to the quality system are "controlled documents". Implementation of the procedure or amendments thereof are under the responsibility of the management representative. The approval authority is as defined in the procedure and the approval of the same authority is necessary for the amendments. Distribution is realized as per the distribution list prepared beforehand. In this way, all the departments are informed duly and on time. Extraction of the obsolete and invalid documents and prevention of their use is thus achieved. All drawings, specifications, standards and similar external documents other than the procedures and work instructions are also within the scope. Regulatory and statutory requirements are monitored by the document control system and implemented as appropriate.

Procurement and material control commences with identification and control of requisition, inquiries and purchase orders and continues through expediting of materials and equipment, through manufacture, shipment and receipt at site. This procedure defines the steps taken to assure the proper execution of these activities, and quality assurance’s role and responsibilities therein. TRIMEC evaluates and selects the subcontractors, vendors and other suppliers according to their past performances, specifications and related technical standards, their technical abilities, quality and price criteria for the activities within the scope of the Quality Plan. The subcontractors and suppliers are evaluated on a systematic and continuous basis in order to improve their quality. Subcontractor assessments are part of the evaluations and are realized as required.

INSPECTION & TESTING For the activities of TRIMEC defined within the scope of this quality manual, the incoming materials and subcontractor services that affect quality are subject to necessary controls in order to achieve the intended conditions. The stages at which controls are performed are as follows: • incoming inspections and testing, • in-process inspections and testing, • final inspection and testing. Where incoming product or the in-process activities is released for urgent production purposes prior to verification; they are positively identified and recorded in order to permit immediate recall and replacement in the event of nonconformity.

CORRECTIVE AND PREVENTIVE ACTIONS In order to implement corrective and preventive actions for existing or potential non conformities and problems, the root cause is examined and analyzed via "teamwork". HANDLING AND STORAGE HANDLING, STORAGE, PACKAGING, PRESERVATION AND DELIVERY All materials within the scope of this Quality are treated according to the safety rules to prevent damage and deterioration during the handling, storage, preservation and delivery. Products are handled and stored in defined stock areas. Stock areas are audited systematically.

QC RECORDS The quality records are kept in order to indicate the suitability and effectiveness of' the quality system related to activities defined within the scope of this Quality Plan. The records of the subcontractors & management reviews are also a part of these data. Some of the records are printed or hand written on paper and some are in the electronic media. In every procedure the relevant quality records, the related authorized personnel and the retention time are defined under the heading of "quality records". The authorized personnel can easily access quality records. The relevant records are presented to the customers, subcontractors and auditors when necessary; which is an indication of the conformity and effectiveness of our quality system. 3.11. AUDITS 3.11.1. Audit Selection The QA/QC Manager will prepare a preliminary list of documents subject to the Quality Assurance audits. Audit Initiation When work has progressed to a selected audit point, the QA/QC Manager or his designee will request the Quality Control checker to make the documents used to perform his checks available for audit. The check copy and checklist should be reviewed immediately. Back-up documentation which is not immediately available should be made available within four hours. 3.11.3. Audit The QA/QC Manager or his designee shall check the documents supplied to assure that all Quality Control functions were performed in accordance with project and TRIMEC Quality Control procedures. The auditor will use the appropriate Quality Assurance checklist. Normally work will not be on hold for an audit. When the audit is complete, all material will be returned to the senders.

Record of Audit At the completion of each audit, the project quality assurance manager shall issue a Record of Audit report, This report shall document what was audited, what reference material was used, what department is responsible for quality control and the findings of the audit. Copies of the report shall be sent to the department head and the Project Manager. 3.11.5. Summary of Audits The Quality Assurance Manager shall maintain a summary of all audits on the project. This summary shall note the item audited, date, responsible department , and the name and date of additional reports filed as a result of the audit. Such reports as a discrepancy report of memo shall be noted. 3.12. TRAINING TRIMEC is determined to improve the talents and expertise of the personnel via training in activities within the scope of this Quality Plan. Apart from the TRIMEC personnel, when necessary, the subcontractor personnel is also considered as "personnel". The personnel are trained according to the requirements of the project and when new equipment or technology is used. The training records are maintained as per the related procedure. The training needs are discussed and approved during the management review meeting; consequently the annual training plan is prepared.

General It is TRIMEC’s stated policy to deliver a quality product, satisfying in all respects the requirements of MTG and the Client. This policy applies to both TRIMEC’s work, and the work performed by its subcontractors, and is carried out through the application of sound engineering, procurement, and construction methods. TRIMEC’s Quality Assurance/Quality Control Program provides a continuing review function to verify that the work is performed in accordance with approved procedures and specifications. QUALITY PLAN Quality Assurance/Quality Control Field Procedures (QA/QC FP) TRIMEC’s QA/AC FP provides a framework for our Quality Assurance/Quality Control Program. It states Company Policy to be adhered to in our areas of operation and provides guidance to our project personnel so that their responsibilities and standards for quality are clear and well defined. It also provides a mechanism to verify that quality conformance is inspected and documented, as required, to the manual standards. The QA/QC FP outlines the areas of administration and project control to provide a comprehensive, total-quality project. The QA/QC FP inspection forms included in the manual serve as a guide for inspection and documentation requirements for each area of Work. These procedures are further developed into detailed, working documents that are project specific, based on the particular project specifications, contract requirements, local jurisdictional authority requirements, and specific vendor recommendations. The procedures and work methods in place cover the areas of work nominally encountered in the construction of an industrial plant. TRIMEC’s QA/QC FP, assigned to each Construction Manager, offers specific guidelines for accomplishing QA/QC activities. TRIMEC’s QA/QC FP is proprietary, and is intended for the sole use of TRIMEC.

Site Monitoring and Program Administration The following summarizes the type of administrative and control functions considered for each major work item associated with a standard industrial plant. Within each area is a general discussion indicating the typical detail considered in developing the project specific QA/QC FP. 4.3. Earthwork and Sitework Quality Control methods for earthwork generally follow the recommendations of the Geotechnical Report; as conveyed in the drawings and specifications. Lines and grades are verified by the survey team and inspection of the materials are carried out by the TRIMEC field staff in conjunction with the Geotechnical Consultant. When applicable, material testing of densities and other pertinent soil data are coordinated through local testing firms. Excavation and backfill placement techniques are monitored by the TRIMEC field staff and documented to ensure conformance to the specifications and safety standards.

Buildings All buildings are designed, fabricated, and built in accordance with the latest edition of the applicable specifications, codes and standards. Building materials, equipment, systems, and appurtenances are inspected upon receipt by the TRIMEC field staff for conformance to specifications. Erection and installation of the materials, equipment, systems and appurtenances are performed in accordance with approves drawings and manufacturer’s recommendations. Equipment and system test are made/or witnessed by the field staff on all installed equipment and systems to verify that building components meet the performance criteria. Tests which may be included are HVAC, electrical, plumbing, communications, and fire protection, and/or any other applicable system. Test results are witnessed, signed off, and recorded.

    121. TRIMEC Engineering & trade ltd.co. QUALITY PLAN 4.5. Structural Steel Structural steel materials are certified as meeting the required sizes and prior to fabrication. Steel fabrication plants are inspected, approved, and periodically audited by the TRIMEC inspection and QA Department. Approved steel fabrication plants have Quality Control Programs for the fabrication and welding of structural steel in place. This Quality Control Program addresses the welding procedures, production techniques, consumable control and fabrication control required to produce steel structures to the project specifications. Fabricated steel is erected in accordance with the approved erection drawings, and the field staff verifies that the correct erection sequence, bolt up, plumbing, and leveling of the steel to the final steel structure is performed. 4.6. Concrete The supply and placement of reinforced concrete are done in accordance with the International Standards and the project specifications. Local batch plants are inspected of their ability to produce quality concrete in conformance with the project specifications. Concrete suppliers are required to have demonstrated Quality Control Program in place for producing consistent, quality concrete. The TRIMEC site staff inspects placing techniques to ensure transportation, consolidation, and protection of the concrete meets project requirements. The TRIMEC site staff will ensure reinforcing steel is of the proper size and grade (from mill certificates) and is accordance with the drawings.

Equipment Equipment that is to be supplied and installed on the project is inspected upon receipt to ensure it meets the requirements set forth in the equipment requisition and purchase order. Where manufacture’s design submissions are required, they are verified as approved by the Engineer. Vendor are also requested to provide proper performance and maintenance data. Equipment storage procedures are maintained in accordance with the vendor requirements and recorded on Material Receiving Reports to verify the condition and temporary maintenance requirements. Installation of equipment follows the Vendor’s suggested procedure, and the equipment is checked for level, alignment, and bolt-down prior to testing. Equipment tests are made and/or witnessed on installed equipment by the TRIMEC field staff to verify the component meets the performance criteria. Such tests may include pressure tests, flushing or purging, run-in tests, verification of operating conditions, or any other applicable tests. The results, along with alignment and bolt-down data, are recorded on the appropriate Quality Assurance document. Where Engineer, Owner, or representative request witnessing, they are notified by the field staff. In the case of subcontracted package work, the Subcontractor must demonstrate an equivalent Quality Assurance/Quality Control Program prior to commencing the work.

Instruments Instrument equipment, materials, and installation are in accordance with the Internationally accepted Codes, or other recommended practices. Vendors of materials and equipment provide design data or material reports for approval prior to supplying components to the project. Material receiving, storage, and installation follows the vendor recommendations and the project specifications. The TRIMEC site staff inspects material and equipment upon receipt, and inspects instruments and control systems during mounting, installation, calibration, and testing. Direct systems tests are made and/or witnessed by the field staff, signed and recorded. The following tests are conducted: hydrostatic testing of instrument air systems, loop checks, and instrument stroking through their entire range and adjustment as necessary. 4.9. Piping The fabrication and installation of piping is carried out in accordance with International Standards and the project specifications. Procedures contained in these documents outline, in detail, the methods of verifying all aspects of pipe fabrication control. Welder qualification and stamp identification are addressed, as are the provisions for job and weld maps of all testing and welding carried out. Project testing requirements will be executed by non-destructive examination testing consultants under the direction of the TRIMEC site staff. The field staff also coordinates verification inspections by the Owner’s representative, when final system testing and sign-off are required.

Vendors of materials and equipment will provide design data or material reports for approval prior to supplying components to the project. Material receiving, storage, and installation follow the standards outlined in the code and the specifications. The TRIMEC site staff inspect installations during construction and direct system testing upon completion. 4.11. Insulation Insulation material, lagging, and fasteners are inspected upon receipt for conformance to the specifications. Materials containers are checked to ensure that they are weather tight, legibly marked, and stored on wood pallets, and sheltered. Insulation materials are installed only during the correct environmental conditions, and are checked prior to lagging to ensure that they are not wet, warped, or damaged. Vessels, tanks, and equipment are checked prior to insulation installation to ensure that correct supports are installed and undamaged. During mechanical completion sign-off, all insulated equipment and piping is checked against the flowsheets and line schedule for correct insulation thickness, type, lagging, and damage.

Storage conditions should follow the manufacturer’s recommendations with particular attention to temperature and pot life. Only approved material applicators mix and apply the materials. Application of the paint or coatings is carried out. A final inspection for appearance, consistency, and dry film thickness is made. 4.13. Quality Audits Quality Audits provide an independent evaluation and check of the quality-related activities and procedures used on MTG projects. Audits are carried out on both TRIMEC’s quality performance and on that of its subcontractors. 4.14. Follow-up Follow-up is essential to verify that commitments to correct deficiencies are made, and that the deficiencies are corrected, The Construction Manager provides initial follow-up. Regular quality audits supplement field control. Knowledgeable personnel are available to our site staff from our Engineering Department for quality problem solving during all stages of construction.
